AFFIDAVIT OF LOSS

I, ARCHIMEDES A. ALDABA, of legal age, Filipino, with residence at 216

San Juan, Malolos, Bulacan, after having been sworn to in accordance with law, hereby
duly depose and say, THAT:

1. I am appointed as Public Attorney in the district of Malolos, Province of Bulacan;

2. Sometime in September of 2007, I was looking for my Public Attorney’s

Identification Card which I usually keep in one of my black leather folder;

3. Earnest efforts were taken on my part in order to locate said Identification Card
but my efforts proved futile;

4. I am executing the foregoing affidavit in order to secure a new Identification Card

from the Public Attorney’s Office;

5. I am attesting to the truthfulness of the foregoing facts and circumstances for

whatever legal purpose that it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 17th day

of October 2007 in the City of Malolos, Philippines.
