AFFIDAVIT
I, JOSE G. DONGON, of legal age, Filipino, married and a resident of San
Nicolas, Iriga City, after having been sworn in accordance with law, do hereby depose
and state that:

1. I am currently employed as driver under contract of service (COS) at the Local

Government Unit of Iriga City (LGU-Iriga);

2. Sometime in afternoon of October 2023, while I was waiting for my porter to

arrive, I decided to park the dump truck of LGU-Iriga that I was driving that
time, near Gaisano Mall, Iriga City.

3. During that time, instead of making a U-turn, I decided to just maneuver the
vehicle in reverse to give the dump truck ample space at the front as I parked it
adjacent to the sidewalk near the Gaisano Mall. In my attempt to slowly back up
the dump truck, I made sure to honk twice to signal any surrounding vehicle.

4. Unknowingly, a white Toyota Wigo Hatchback was already parked behind me,
and did not hear the signaling sound resulting to bumping and damaging the
front part of the said Wigo car.

5. After the said accident, I immediately apologized to avoid any further conflict
with the driver of Wigo car.

6. I am executing this Affidavit to certify or attest to the truth of the foregoing facts
and for whatever legal purpose it may serve.
