AFFIDAVIT OF GUARDIANSHIP

I, GERMAN C. ALMELOR of legal age, Filipino, and presently residing at Sta.

Cruz Sur, Iriga City, after having been duly sworn to in accordance with law, do hereby
depose and say that:

1. I am the biological father of RIZALYN M. ALMELOR (Rizalyn). The whereabouts of

her biological mother, MIRASOL D. MURILLO, are unknown;

2. My daughter Rizalyn is now under my care and custody;

3. I hereby declare myself as the de facto guardian of my daughter, Rizalyn; and

4. That I am executing this affidavit to attest to the truth of all the foregoing
statements and for whatever legal purpose it may serve.
