A RESOLUTION CONFIRMING THE APPOINTMENT OF name of secretary as BARANGAY SECREATRY and

name of treasurer as BARANGAY TREASURER OF BARANGAY _________LA PAZ, TARLAC:

WHEREAS ; Punong Barangay name of PB must have to appoint a Barangay Secretyary and Treasurer
with the concurrence of the majority of the Sangguniang Barangay members;

WHEREA; Punong Barangay ____________ and the Sangguniang Barangay members agreed that they
will appoint ___________________as Brgy. Secretary and ________________as Brgy. Treasurer;

WHEREAS; _________________ will perform such duties as Barangay Secretary and ___________ as
Barangay Treasurer effective Nov.__________________, 2023.

RESOLVED FURTHER, that copy of this Resolution be furnish to the Local Office of DILG and other
concerned for their information
