AFFIDAVIT OF LOSS

I, NOLITO A. DEFINO, of legal age, Filipino, single and a resident #5 Romantic

Village, Brgy. Sta. Ana, Taguig City, Philippines, after having been duly sworn in
accordance with law, hereby depose and state that:

1. While I was checking my personal belongings, I noticed that my

Professional Driver’s License numbered N5019013100 which is valid till
08/11/2024 is missing;

2. I have no idea of its loss as to whether it was dropped, misplaced;

3. I exerted diligent efforts to locate the same but my efforts turned fruitless
and to this date said Professional Driver’s license is still missing hence, it
is now safe to declare it as lost and beyond hope of recovery;

4. Said Professional driver’s license was not confiscated for violation of any
law or rules and regulations and hereby request the office of the Land
Transportation Office to issue a duplicate/new license in lieu of the lost
one;

5. That I executed this affidavit to attest and confirm the foregoing facts and
for whatever legal purpose it may serve best;
