2. NEXUS ALLIANCE LIMITED HSE MANAGEMENT SYSTEM (HSE MS)

NEXUS ALLIANCE LIMITED HSE Management System (HSE MS) is


presented below. This System provides a detailed and organized approach
to HSE Management. NEXUS ALLIANCE LIMITED is of the belief that a
good HSE performance is a reflection of a sound HSE Management
System.

2.1 LEADERSHIP AND COMMITMENT

NEXUS ALLIANCE LIMITED Senior Management shall provide


strong, visible leadership and commitment to HSE Management
through the following means:

 Provision of adequate resources, competent personnel and fit for


purpose equipment for the execution of any Contract.

 Involvement in and chairing of HSE Meeting:

- Site HSE Meetings shall be chaired by an Executive


Director or his designated Project Manager. This shall
be held once a week.

- Management HSE Performance Review Meetings shall


be chaired by the Managing Director. The frequency
shall be quarterly.

 Involvement in site visits, once in every two months.

 Involvement in Management Facility Inspections.

 Promotion of HSE issues in the organisations.

 Involvement in HSE inspections and audits.

 Involvement in training.

 Involvement in Project Workshop in which HSE issues are also


part of the agenda, once in every three months.

2.2 POLICY AND STRATEGIC OBJECTIVES

NEXUS ALLIANCE LIMITED HSE Policy and Strategic Objectives are:

 Consistent with international standard

 Relevant to NEXUS ALLIANCE LIMITED activities and services.

 Consistent with, and are of equal importance to NEXUS ALLIANCE
LIMITED other business policies and strategic objectives.

 Publicly available.

 Formulated to meet all relevant regulatory and legislative


requirements.

 Formulated to reduce the risks and hazards to health, safety and


the environment to levels, which are as low as reasonably
practicable.

 Devised to provide a framework for setting and reviewing HSE


objectives and targets for a continuous improvement of HSE
performance

2.3 ORGANISATION AND RESPONSIBILITIES

NEXUS ALLIANCE LIMITED shall develop a system of HSE-


dedicated meetings / committees to review and communicate HSE
issues to all level. The meetings / committees shall interface with
business management meetings / committees at the same level of
NEXUS ALLIANCE LIMITED organization.

These meetings / committees shall be organised in such a manner as


to provide a forum for HSE information dissemination to all personnel
in terms of HSE policy and HSE management requirements in addition
to the roles and responsibilities of every personnel with respect to
HSE Management. HSE shall be the responsibility of line
management at all levels of NEXUS ALLIANCE LIMITED organisation.

2.4 MANPOWER RESOURCES AND COMPETENCE ASSURANCE

NEXUS ALLIANCE LIMITED Senior Management shall allocate


sufficient resources at all levels of HSE Management. Every
personnel as well as Sub-Contractors must be competent to perform
his assigned Project HSE duties. HSE training shall be provided to
employees.

2.5 HAZARDS AND EFFECTS MANAGEMENT

NEXUS ALLIANCE LIMITED will carry out assessment of Project


works and services to identify HSE hazards and effects, evaluate the
significance of these HSE effects, assess the adequacy of the
controls, and develop / implement management plans to minimise
these effects to a practicable extent.

2.6 PLANNING

As part of NEXUS ALLIANCE LIMITED HSE Management Plans,


NEXUS ALLIANCE LIMITED Management shall allocate sufficient
human, physical and financial resources towards improving HSE
performance. NEXUS ALLIANCE LIMITED shall develop HSE
objectives and targets, Emergency Response Plans and Procedures
and other specific Procedures and communicate it to all workers:

2.7 STANDARDS, PROCEDURES AND DOCUMENT CONTROL

The Procedures and Plans required shall be in compliance with our


Client‟s HSE legal requirements, HSE standards, international HSE
standards and local laws. The procedures shall cover all the phases
of our activities. HSE documents shall be controlled.

3. IMPLEMENTATION AND PERFORMANCE MONITORING

HSE performance will be continuously monitored through statistical


methods. NEXUS ALLIANCE LIMITED shall develop procedures for
incident/accident notification, reporting and follow-up, HSE meetings,
toolbox talks and notification/reporting and management of non-
conformances in conformance with international and local standards.

3.1 AUDIT

NEXUS ALLIANCE LIMITED shall develop and implement an HSE


Audit Program and follow-up process.

3.2 MANAGEMENT REVIEW

NEXUS ALLIANCE LIMITED Senior Management shall review the


HSE Management System, Policies and Strategic Objectives at
Corporate level in order to ensure their continuing adequacy and
effectiveness once in every three months. Management review shall
also be carried out on HSE targets and objectives in order to
compare actual set targets and objectives.

4. ORGANISATION, RESPONSIBILITIES, RESOURCES, STANDARDS

4.4 NEXUS ALLIANCE LIMITED Management Committee

 They have overall responsibility for all HSE matters within the
Corporate Organisation.

 They have the responsibility to regularly review the


effectiveness of the policies and the personnel under their
control onto whom responsibilities for the various aspects of
HSE matters have been assigned.

 They have responsibility to ensure that sufficient funds, staff


and materials are available to meet HSE Standards on Projects
and at corporate levels.

NEXUS ALLIANCE LIMITED Management Committee include:

- Managing Director.

- Manager - Commercial/ Business Development

- Manager - Technical Services

- Manager – Human Resources

- Officer – HSE

4.4.1 Managing Director

The Managing Director is responsible for discharging


NEXUS ALLIANCE LIMITED HSE Policies.

He is primarily responsible for delivering the „safety first‟


work ethics and culture of the organization. In addition, it
remains his duty to ensure that:

 Employees receive sufficient information, Instruction,


Training and Supervision.

 NEXUS ALLIANCE LIMITED HSE Management


System is periodically appraised and if necessary
revised.

4.4.2 Technical / Commercial/Business Development / Human


Resources Manager

These Managers shall ensure that all activities under their


control are carried out in accordance with NEXUS
ALLIANCE LIMITED and Client HSE Policies.

They may formally delegate specific responsibilities to
selected persons. However, they shall be directly
responsible for the following:

 The adoption of safe working procedures including


safe place of work.

 The establishment of HSE Management System


implementation programme.

 Auditing of programmes and plans to measure


progress and identification of deficiencies.

 Adequate training of personnel to improve their safety


at work.

 That Selected Sub-Contractors are able to meet


NEXUS ALLIANCE LIMITED standards.

 That deficiency in equipment, standards, operating


procedures or training is corrected.

 Promotion of a high degree of safety and


environmental awareness by personal example.

 That all accidents/incidents are reported and


investigated according to NEXUS ALLIANCE LIMITED
procedures and Client requirements.

4.4.3 HSE Officer

The HSE Officer shall:

 Have direct responsibility for all HSE issues and shall


report directly to the Managing Director.

 Prepare HSE Plans and HSE Rules.

 Manage the HSE functions.

 Maintain a HSE information service and dissemination


of HSE information, as well as learning points from
accidents/incidents to the workforce.

 Monitor the effectiveness of the HSE Management


System.

 Liaise regularly with Site Safety Representatives.

 Support and advise the Project team on health and


safety of personnel.

 Introduce Safety notices, bulletins and posters.

 Conduct Safety audits in conjunction with


management and ensure that HSE information from is
available to the Manager/Director for review at the
HSE Review Meetings.

 Contribute to the planning and implementation of risk


assessment and control measures and contribute to
the implementation of selected risk control and
mitigation measures.

 Revise the HSE Plan and other required Project HSE


procedures when necessary.

 Review standards and procedures in conjunction with


Departmental Managers in order to improve HSE.

 Attend all HSE meetings with the Managing Director


and assist in such areas as action plans for accident
prevention and the setting of realistic HSE targets.

 Develop emergency response procedures for and


ensure that these are communicated to the workforce.

 Be involved in accident investigation and reporting and


maintain a register of accidents/incidents and unsafe
acts and conditions.

 Review all accidents/incidents report and remedial


actions.

 Liaise with Client HSE Representatives.

 Liaise with Government Agency Representatives on


HSE matters.

 Liaise with relevant personnel as required, such as


Line Managers, Supervisors, Safety Committee and
assigned Emergency Response team.

4.4.4 Supervisors

Supervisors are directly responsible to their Managers for


the day-to-day maintenance of safe working operations at
the place of work.

Direct responsibilities include ensuring:

 The promotion of safe working practices and safe


place of work by personal example.

 That selected personnel are competent to carry out


the task.

 That pre-job description/discussion is held before the


start of the daily activity and that adequate supervision
is maintained during work task.

 That formal procedure, standing instructions and


safety standards are complied with at all times.

 That individual employees are aware of their


responsibilities and are accountable for them.

 Review of details of every task to be performed with


Managers, other Supervisors, and individuals as
appropriate, to ensure all factors likely to affect safety
have been fully considered and appropriate action
taken.

 All accidents, incidents and non-conformances are


reported and remedial actions immediately applied
where practicable.

 That the workforce participates in drills, audits, HSE


Meetings, HSE Training and other HSE Programmes

4.4.5 Employees

Employees at all levels are responsible to their


Management for the execution of the HSE Policy within
their sphere of operations, and for reporting on the
effectiveness of the steps taken to achieve the Policy
Objectives.

It is the statutory duty of every employee to take, all


reasonable care, of his or her Health and Safety, and of

any other persons who may be affected by his/her acts at
work. All employees are encouraged to co-operate with
their Management and to report malpractices and
deficiencies in plant or procedures in order that corrective
action can be taken to enhance the Safety of the
workplace. This shall be achieved via the HSE Review
Meetings. In addition, responsibilities of each employee
shall include the following:

 Carrying out activities in accordance with NEXUS


ALLIANCE LIMITED Standards and Procedures as
well as CLIENT HSE Standards, Policies and
Procedures, which shall be disseminated to them
during HSE inductions. Employee compliance with
these Policies and Procedures shall be monitored
through audits.

 Complying with statutory obligations imposed by


Nigerian legislation.

 Co-operating fully with Supervisors or Managers in the


promotion of safety.

 Becoming familiar with any information published by


NEXUS ALLIANCE LIMITED or brought to their notice
regarding the safe use of equipment, plant and
materials relevant to their work.

 Reporting any non-conformances or potential hazards


to their supervisor.

 Participating in Emergency Response Drills as


required.

 Every employee must not:

 Participate in any activity, which he/she believes may


endanger himself/herself or his/her colleagues.

 Jeopardize the safety and integrity of NEXUS


ALLIANCE LIMITED equipment or facilities including
the misuse of emergency equipment.

 Undertake any task or operation for which he/she


lacks proper knowledge, tools, equipment or personal
protective equipment.

 Operate plant and/or equipment, which he/she knows
to be inadequately guarded or unsafe.

4.4.6 Sub-Contractors’ HSE Focal Points

Sub-Contractors‟ HSE Focal Points shall report to NEXUS


ALLIANCE LIMITED HSE officer on all HSE Issues on the
Project.

6. Manning / Communications

HSE Issues shall be communicated and disseminated to NEXUS ALLIANCE


LIMITED and Sub-Contractor‟s workforce through the following methods: -

 Personal contact (instructing the worker in the language he/she understands, for
example, native language or pidgin English)

 Notice boards,

 HSE Meetings,

 Monthly Newsletters,

 Poster Campaigns,

 Video Clips,

 HSE Training Sessions

 HSE Information posted on notice boards - Notice boards shall be positioned at


strategic areas.

NEXUS ALLIANCE LIMITED shall communicate HSE issues with CLIENT through:

 Monthly HSE Review Meetings between CLIENT Representatives and NEXUS


ALLIANCE LIMITED team.

 Ad-hoc HSE Meetings between NEXUS ALLIANCE LIMITED and CLIENT to


address critical jobs.

 HSE Reports: This will be included in the Project Monthly Report to CLIENT.

6.1 HSE Professionals

6.1.1 Reporting and Follow up

Evidence of how line management follows advice offered by HSE


Professionals shall be visible through the interplay between HSE
personnel and line Management.

6.1.2 HSE Officer

The HSE Officer shall be involved in the following:

 Preparation of HSE Plan.

 Preparing and monitoring of departmental action plans.

 Formulation of HSE rules suitable to the Project

 Inspection and Audits.

 Promotional Material.

 Training.

 Sub-Contractor assessment.

 Health risk assessment, performance monitoring and


surveillance.

 Supporting of incident/accident investigation.

NEXUS ALLIANCE LIMITED Management shall be involved in


preparing and implementing the under-listed functions, which
however, could be delegated to the HSE Officer but final approval
must come from NEXUS ALLIANCE LIMITED Managing Director.

 Emergency manuals and plans

 Training for fire fighting teams, first aid proficiency, etc

 Emergency drills/exercises

 Provision and Use of Protective equipment.

7. Sub-Contractors

7.1 Management

Sub-Contractors form an integral part of NEXUS ALLIANCE LIMITED HSE


Organization and shall be included in the overall HSE Management,
procedures, statistics, training, reporting of performance, meetings,
communication and promotions, inspection and audits. All Sub-Contractor‟s
will be pre-qualified by NEXUS ALLIANCE LIMITED against HSE standards
before contract award. Records of these pre-qualifications shall be maintained
by the HSE Department.

The awarding of contracts by NEXUS ALLIANCE LIMITED shall be


determined not only on the grounds of prices and technical ability but also on

past records and present ability to carry out the work safely and without risk to
the health of personnel and the protection of the environment.

The Sub-Contractor‟s HSE Management structure shall be assessed by NEXUS


ALLIANCE LIMITED Management to include the following:

a) HSE Policy Statement.

b) HSE Performance.

c) Details of Personnel experience.

d) Details of Personnel qualifications.

e) Safe Working Procedures.

f) Emergency Response Procedures.

g) Personal Protective Equipment Procedures.

h) Sub-Contractors‟ Insurance Policies.

8. HSE Communications

8.1 Coverage / Awareness

NEXUS ALLIANCE LIMITED shall maintain a sound communication system to


enhance fast reporting of HSE issues to the key persons required for HSE
Management and to CLIENT. Mobile Telephones, Fixed Line Telephones, fax
machines, radios (fixed types or hand-held types) shall be maintained, as
applicable, at the various facilities where NEXUS ALLIANCE staff work.

Major incidents that may endanger personnel and which could require the
support of established external agencies/organisations are:

 Medical emergencies beyond the scope of a work site or facility.

 Major vehicle accidents.

The organisation that would be expected to provide support in a major


medical emergency is NEXUS ALLIANCE LIMITED retainer clinics.

9. HSE Meeting Programme

9.1 Scheduling

Below is a schedule of HSE Meetings. The meetings will be held as stated


below:

 CLIENT / NEXUS ALLIANCE LIMITED HSE Meetings

This will be held on a quarterly basis.

 NEXUS ALLIANCE LIMITED HSE Meetings

This will be held with the entire workforce on a monthly basis

 Craft Specific HSE Meetings

This will be held on a weekly basis

 Toolbox Meetings

This will be held on a daily basis, before start of the day‟s activity.

In addition to the above HSE Meetings, NEXUS ALLIANCE LIMITED shall


hold Ad-hoc HSE Meetings with CLIENT prior to the start of potentially
dangerous activities.

During the meetings, an attendance sheet will be passed round. Everyone in


attendance will write and sign his name on the sheet. The attendance sheet
will be attached to the HSE Meeting Reports.

9.2 Management Participation

Management shall participate in all HSE activities. Management will


disseminate HSE objectives as set by them to the workers during HSE
Meetings. Monitoring of HSE performance through audits and inspections
and support for NEXUS ALLIANCE LIMITED policies and objectives shall be
part of Management responsibility. Management shall be involved in a
Quarterly facility Visit, during which they will walk around and ask interactive
questions to the workers to get to know HSE issues requiring immediate
Management support.

9.3 HSE Meeting Structure

Typical agenda for HSE Meetings are provided below.

9.3.3 Follow Up Actions

Follow up actions arising from HSE Meetings shall be documented.


Target date for completion will be set and dedicated Personnel

9.4 Communication

NEXUS ALLIANCE LIMITED shall demonstrate that:

 HSE issues are communicated to Workers.

 Meeting Programme is consistent with the rest of management


structure.

 HSE Meetings are recorded, through the issuing of Minutes of Meetings.

10. HSE Promotion and Awareness

10.1 Techniques

Appropriate HSE communication through which HSE issues shall be


communicated to the workforce.

10.2 Performance

NEXUS ALLIANCE LIMITED shall provide HSE performance boards, where
all HSE statistics are displayed.

10.3 Promotional Methods

HSE performance of staff or work teams will be closely monitored and they
will be encouraged to achieve better performance through incentives, such as
Safety Awards, small “give-aways ” with HSE message.

The HSE Committee shall determine the type of awards and the frequency.
Incentive Programme shall be part of the agenda for HSE Review Meetings.

11. HSE Competency Requirements

11.1 Fitness of Personnel

All Personnel, including Sub-Contractors shall be certified medically fit to work


in line with NEXUS ALLIANCE LIMITED Policy and local laws. The medical
fitness test shall be carried out in NEXUS ALLIANCE LIMITED retainer clinics.
A database of all Personnel shall be maintained by the HSE Officer and shall
contain medical certificate number, date of certification and certifying clinic.
Medical records for all work-related injuries/illnesses shall be kept for all
Personnel as applicable.

11.2 Employee Orientation Programme

11.2.1 Approach

NEXUS ALLIANCE LIMITED shall carry out an on-the-job


orientation for supervisory staff as follows:

 Upon engagement of a supervisory staff, the staff shall


report to his direct sectional Manager to get his job
briefing, posting and his team.

 Thereafter his sectional head will conduct him round the


company emphasizing on:

 Muster point location

 HSE meeting venues

 Emergency procedures

 Restricted Areas

 Facility/Site Rules

 PPE requirements, etc.

 Incident/accident reporting

 NEXUS ALLIANCE LIMITED HSE Policy and CLIENT


HSE Policy.

11.3 New Employees

New employees are the most vulnerable on work sites. NEXUS ALLIANCE
LIMITED has a system in place to ensure that new employees are adequately
trained and are coached to improve their working practices.

 Upon engagement of new employees on the Project, they shall


undergo an HSE Induction-training programme carried out by the
HSE Officer, after which they are handed over to their direct
supervisor.

 If required, it is the responsibility of their direct supervisor to take


them to the store to obtain their PPE.

 Thereafter the supervisor shall brief them about the job, the
hazards, which their jobs can expose them to, and the means of
preventing them as well as the HSE requirements on the job.

 The supervisor shall closely monitor them while they are working to
ensure that they are following approved work methods.

 He shall correct them in the event that they are not following
approved work methods in order to improve their working practices.

12. HSE Training (General)

NEXUS ALLIANCE LIMITED Employees, including Sub-Contractor personnel must


be sufficiently aware, informed and trained to perform their daily activities in
accordance with NEXUS ALLIANCE LIMITED HSE Policies. It is Management‟s
belief that staff who have acquired training in HSE Programmes as well as in their
trade are invaluable assets to NEXUS ALLIANCE LIMITED as the application of
experience gathered during training programmes help to minimize accidents.

NEXUS ALLIANCE LIMITED Employees shall not only be experienced and trained
to perform their job but shall also be experienced in HSE issues. In order to improve
their awareness and training on HSE issues, NEXUS ALLIANCE LIMITED shall
identify training needs for the various categories of staff in order to meet the required
standard.

12.1 Formalised Programme

NEXUS ALLIANCE LIMITED shall maintain a matrix defining training needs
for Personnel and records of employees who have been through the training
programme. The matrix shall contain the qualifications gained by the
employees and the extent of coverage of the training courses.

NEXUS ALLIANCE LIMITED employees shall be trained and qualified in their


respective trades before starting work.

12.2 HSE Training (Professionals)

NEXUS ALLIANCE LIMITED shall utilise advice of their in-house and external
HSE Professionals as may be required in order to work in line with relevant
local laws and minimise HSE infringements.

Selection of HSE supervisory staff is based on experience in the Project


related activities (at least five year experience) and membership of the HSE
supervisory staff of any of the following:

IOSH - Institution of Occupational Safety and Health

NISP - Nigerian Institute of Safety Professionals

RSP - Registered Safety Professional

DIPSM - diploma in SAFETY management or Member of International


Institute of Safety Management

The HSE supervisory staff must have undergone HSE trainings that conform
to Client and NEXUS ALLIANCE LIMITED requirements (Minimum of level 3
of NISP)

13. HSE Legislation

13.1 Coverage

This HSE Plan is based on the following laws, and regulations, standards and
codes of practice, given below in order of hierarchical precedence, relating to
HSE.

13.1.1 International standards

 UK Health and Safety at Work Act 1974.

 Occupational Safety & Health Administration (OSHA) Standards.

 ANSI Standards.

 ISO 14001.

 Guidelines and Standards for Environmental Pollution Control in


Nigeria (1991)

13.1.3 Client Specifications

NEXUS ALLIANCE LIMITED shall comply with Client


Specifications.

14. HSE Standards

14.1 Availability

NEXUS ALLIANCE LIMITED HSE procedures, standards, rules and


regulations are contained in NEXUS ALLIANCE LIMITED HSE Manual. The
HSE manual shall be issued to every Personnel during induction in order to
make him/her familiar with HSE procedures, standards, rules and regulations.

14.2 Permit To Work

It is the responsibility of NEXUS ALLIANCE LIMITED project Supervisor to


ensure that relevant work permits are obtained from CLIENT for the
performance of the works throughout the duration of the work in all areas
controlled by CLIENT. NEXUS ALLIANCE LIMITED shall abide by the safety
instructions contained in Client Permit To Work.

NEXUS ALLIANCE LIMITED shall abide by its own Work Permits when
working in its own facilities.

15. HAZARDS AND EFFECTS MANAGEMENT

15.1 Coverage

NEXUS ALLIANCE LIMITED shall carry out generic risk assessment/task
specific risk analysis on all tasks before starting work. All recommendation
must be fully complied with, and shall be a condition for continuing work.

15.1.1 Handling of Chemicals and Other Potential Hazards

15.1.2 Coverage

NEXUS ALLIANCE LIMITED shall ensure that Personnel exposed


to chemicals shall be made aware of chemical health hazards.
NEXUS ALLIANCE LIMITED shall also provide adequate protection
for those exposed to the effects of chemicals, and other products,
which can occur through inhalation or ingestion or through direct
contact with the skin. The chemical, physical and toxicological
characteristics of the chemical shall be explained to the workers.

Staff awareness of hazards associated with chemicals shall be


achieved through training, health education, safety meetings, and
safety posters. Personnel exposed to the hazards of chemicals
shall be guided on the contents of the MSDS through their
supervisors.

16. Basic HSE Rules

NEXUS ALLIANCE LIMITED shall provide an HSE manual to every Personnel. It


shall be issued to new employees during their HSE Inductions, i.e. before they start
work. Content of the manual shall be discussed with Personnel during HSE
Inductions. Topics for General HSE Meetings and toolbox meetings will be obtained
from the HSE Manual.

17. Emergency Response Procedures

Various emergency response scenarios have been identified and their response
patterns are as shown below. Adequate resources, material and trained personnel
shall be made available by NEXUS ALLIANCE LIMITED to ensure the
implementation of a sound emergency response should the need arise.

17.1 Safety Emergencies

17.1.1 Fire Emergency

In general, on noticing fire, Employees shall follow the procedure


below:

 Do not panic.

 Sound the alarm by activating the fire alarm, or by shouting Fire!


Fire!!, fire!!! Or by ringing a hand bell, whichever is applicable.

 Fire wardens proceed to the scene of fire with potable fire
extinguishers.

 Other workers shall briskly walk to the designated mustering


point and await further instructions.

 Muster point leader shall carry out head count.

 Search Party shall check that no person is trapped.

 Workers shall be de-briefed after the fire has been put out and
shall be asked to return to their places of work after the all-clear
alarm.

17.2 Medical Emergencies

17.2.1 Medevac

In case of Medical Emergencies beyond the scope of the facility/Site


first aid giver, the Medevac Procedure below shall apply.

The following should be noted:

 NEXUS ALLIANCE LIMITED Base Office and the Retainer-ship


Clinic shall be contacted to make necessary arrangements to
receive the injured person.

 The HSE Officer shall accompany the injured worker from the
facility/site to the receiving Client or NEXUS ALLIANCE
LIMITED Retainer-ship Clinic.

In general personnel shall be educated on the Basic Principles to


follow in case of Medical Emergencies as shown below.

to check if it is safe.

17.3 Total Evacuation From Site

Some security emergencies have the potential to degenerate to a level such


that a total evacuation of personnel will become inevitable. Such
emergencies include:

 Outbreak of Hostilities (Civil Unrest).

 Protracted attack by robbers.

 Protracted Community Disturbance.

18. Emergency Drills

Drills shall be planned and executed to test the effectiveness of the emergency
procedures.

Personnel shall be trained on the various alarm signals and response patterns. Drills
shall be carried out without warning. It is the responsibility of the HSE Officer to
report the drill to the Managing Director after the drill review team has reviewed it.
The drill review team shall follow up the recommendations of the drill to ensure their
implementation. The drill review team shall comprise:

 The HSE Officer

 Human Resources Manager

 A Supervisor

 Selected workers.

19. HSE Inspection (Site Based Projects)

HSE Inspection of equipment shall be carried out on, vehicles and all powered
equipment and machinery

Jobsite inspections by the HSE Officer and Supervisors shall also be carried out on a
daily basis.

19.1 Pre-mobilisation Inspection

NEXUS ALLIANCE LIMITED shall inspect all equipment required for work.
This is to ensure that all pieces of equipment to be used are in good working
condition. NEXUS ALLIANCE LIMITED shall also ensure that all pre-
mobilized pieces of equipment are maintained in a serviceable condition
throughout the duration of the work.

Joint Pre-Mobilisation inspection may be conducted by NEXUS ALLIANCE


LIMITED and CLIENT. NEXUS ALLIANCE LIMITED shall formally inform the
CLIENT Representative at least two weeks in advance stating the types,
serial numbers, models and locations of the equipment. A list shall be kept on
site /office for all pre-mobilized and approved equipment as well as their pre-
mobilization certificates.

19.2 Re-Inspection of Equipment

This requires a re-inspection of equipment at the expiration of the pre-


mobilisation certificates.

19.3 Daily Equipment Inspection

All operators of forklifts, cranes, excavators etc as well as drivers must check
their machines every morning before the start of the day‟s activity for water,
oil, and general condition of equipment etc and report any faults noticed to
their supervisor. These checks shall be logged in the logbooks provided for
them.

20. Occupational Health

20.1 Facilities Available

NEXUS ALLIANCE LIMITED maintains base clinic and ensure that there is
adequate supply of drugs for the day-to-day needs of the staff.

Adequately trained, experienced, registered staff, in accordance with relevant


local laws, shall man the clinic.

20.1.1 General

 Portable fire extinguishers shall be provided in the office
blocks/workshops area and maintained in a serviceable
condition throughout the duration of the project.

 Waste bins /Skids shall be provided for collection of waste. All


waste generated shall be managed properly, through collection,
segregation and disposal

 Lighting shall be provided via standby generators to ensure an


adequate illumination of the office and the fabrication yards.
Regular spraying with insecticide and/or fumigation shall be
embarked upon.

 “No Smoking Signs” shall be displayed and enforced.

20.1.2 Hygiene and House keeping

Good sewage facility /cloak room shall be provide for personnel


use, and shall be kept clean at all times by the attendants.
Housekeeping is continuous throughout the duration of work.

21. Road and Marine Transport

21.1 Drivers – Competence and Selection

Drivers shall be suitably qualified and must possess applicable National


Drivers Licence. In addition to possessing the National Drivers Licence,
NEXUS ALLIANCE LIMITED shall test all drivers before assigning them to a
vehicle upon successful completion of Driving Test conducted by NEXUS
ALLIANCE LIMITED Limited. Experienced drivers will be used.

Drivers must be medically checked to ensure that they are fit before their final
engagement by NEXUS ALLIANCE LIMITED Limited.

21.2 Operations Management

NEXUS ALLIANCE LIMITED shall maintain Journey Management system in


compliance with national legislation. The system shall establish justification
for journey, awareness of route, verification of driver standards and realistic
schedules.

21.3 Contracting

Sub-contractors must implement NEXUS ALLIANCE LIMITED Journey


Management system.

NEXUS ALLIANCE LIMITED has an Incident / Accident Reporting,


Investigation and Follow up procedure.

All accidents/near miss must be reported within 24 hours of occurrence. An


investigation team shall be constituted to unearth the root cause of the

accident. Findings shall be communicated to the workforce during meeting to
create awareness.

23. AUDIT, REVIEW AND INSPECTION

23.1 Scope

The Audit and Inspection shall include the following.:

 Occupational Health

 Unsafe acts

 Audit Training

 Environmental Performance/Compliant

 NEXUS ALLIANCE LIMITED activities and those of Sub-

23.2 Team Membership/Composition

Team members of NEXUS ALLIANCE LIMITED Audit / Inspection Plan shall


involve those outside the Audit location such as CLIENT, government
Agencies when required and Sub-Contractor personnel.

23.3 Effectiveness

Corporate Management shall be involved in the review of Audit / Inspection


findings. This shall form part of the agenda in the Management Review
Meetings. Lessons learnt from the audits/inspections shall be used to
improve operations through the incorporation of recommendations in activity
planning and work methods.

23.4 Follow Up

On the audit/inspection findings report, a date shall be fixed for a follow up.
The HSE/QA Adviser shall keep a database of all audit/inspections findings.
