I believe it was human error that caused the club fire in the Philippines
way back year 1996. The owner of the building had overlooked lots of
important things that should be considered first thing in constructing a
building. Based on the investigation done by Col. Sammy Tadeo, they
found out several factors that triggered the ignition of fire. The absence
of fire exit in the disco club made the situation even worse. It was said
that there are other establishments beside the club that blocked the
possible fire exit. Another factor is the way the door in the club was built.
As per law, it is required for doors to be opened both inward and outward
for safety purposes however in the Ozone Disco Club it had a swing-in
door. When the people started panicking they tried to push the door for
their way out which is supposedly could be opened inward making it
impossible for them to open and get out. The overcapacity of people was
also considered as one of the factors. The disco room could only occupy
around 100 people however on that particular time they catered more
than 300 people. If these factors were not overlooked by the owners in
the first place, it would probably not end as worse as it was.

2.) For illustration, kindly refer to explanation box.

The three components in order to ignite fire are heat, fuel
and oxygen. In the case of the Ozone Disco Club incident, the factors that
influenced the retention of heat was the closed room area and the
concrete wall. The fuel source was the electrical circuit issue that
causes spark in the booth then eventually led to fire and spread through
the ceiling. Third is the oxygen or air ventilation that was influenced by
the overcapacity of people inside the room and the low ventilation
because of less spaces inside. These three components ignited and
sustained the fire inside the disco room that made the tragedy as the
worst club fire in the Philippines.

3.)
An accident such as fire is inevitable however we can do
something to prevent the worst possible damaged it may cause. In
preventing fire accident, it should starts within us. In order to improve our
learning for safety we should educate ourselves with the community
standards, life safety systems and attend seminars related with these. As
much as possible we need to be responsible in our actions like knowing
the emergency exits, fire escapes, fire alarm, and fire extinguishers. This
kind of incident can be prevented for as long as there is cooperative
education, responsible for every action, and seriousness in adhering to
safety precautions and protocols.

4.)
If I belong to the group of survivors, I think the main reason
why they survived was that God gave them a second chance to live. Other
reasons that made them escaped from the fire incident could probably be
the presence of mind in the midst of the commotion, looking for possible
exit without panicking, and communicating with others to find better
solutions. There could also be other survivors who had a strong intuition
and think in advance to immediately go outside the club the moment they
noticed a spark at the booth.

5.)
If ever I will encounter similar incident to the Ozone Disco
Tragedy, the lessons that I learned from it that I could apply are being
responsible and extra cautious before engaging to any activities. I should
be responsible enough to do some research on how to deal with fire
incident to acquire knowledge in dealing those difficult situation. Being
extra cautious is also a must, like familiarizing the map to know where
the fire exit or the emergency exit is and know how to use the fire
extinguisher just incase. I also learned that staying calm while thinking of
possible solutions will increase your chance of living. Panicking will not
buy you some time instead it will only complicate the situation. We must
then be responsible in every actions that we take to prevent the incident
or preventing to be the cause of it.
