Termination of cables should be in water proof
termination box and electrical tapes should not be
used in exposed environment as it will deteriorate
over time.
Over head electrical cable should be supported
higher than the tallest person to prevent electrical
hazard and strangulation to person walking at night
as the place is not lighted.
Fluorescent lamp was filled with dead insects and
there was water ingress. The ingress of insect and
water is normally thru under size cable at the cable
gland or other gaps. Therefore these gaps should be
rectified or sealed. There should be periodic
inspection of all the lamps and during such
inspection, such issues should be arrested before it
causes the lamp to fail.
Electrical panel should be labelled to prevent
inadvertent operation.
Electrical switch gear room should have rubber
insulation mat at the panel floor.

All oil booms found kept below the jetty platform in

barnacles on the piles during the change of tide and
movement of the current, the boom body will get

can be easily and quickly be deployed into the water

Outside the perimeter of the bund wall, grass should
be trim short so that it will not be a hinderance
when there is emergency getting fire hoses to these
area. Water ponding should be easily leveled up with
soil to prevent slip trip and fall accessing these area
in the dark and in an emergency. In rainy weather,
these pond could be enlarged.
Isolated concrete structure in the middle of the field,
internal stagnated water should be drained to
prevent the breeding of mosquitoes. Unwanted
items should be disposed off.
Fire hydrant should be capped when not in use to
prevent mechanical damage to the connection lips.

Terminal operator and surveyor should exercise
good personal habits by not throwing unwanted
items everywhere on the ground.
Used seals, empty plastic bottles and rags are found
thrown on the floor in the tank farm area. These
habit should be improved with good practice of
house keeping. Every terminal operator and
surveyors should exercise owner ship of the location
they work in and dispose litter in the correct
location.
Items in LOTTO box should be better organized. It
can be seen that the LOTO items were not used.

There are only 2 flood lamps covering the diesel tank
farm tanks on seaward side of the perimeter bund
wall covering 8 shore tanks.
There are also no flood lamps at the diesel tank top.
Most of the area will be dark by night beside the 2
flood lamps.
Although handy torch lights are used at night,
consider to put in place sufficient lighting to prevent
slip, trip and fall hazard during night operation.
