AFFIDAVIT OF SUPPORT
I, ADRIAN M. CRUZ, Filipino, of legal age, and resident of 5 Tropical Drive, Maries
Village, Quezon City, after having been duly sworn in accordance with law, hereby depose and
state that:

1. I am the uncle of Jann Andreii C. Cruz who is the son of my brother Aurelio M. Cruz, Jr.,
both citizens of the Republic of the Philippines and are joining me and other members of
the family in a trip to Japan from January 2 to 7, 2019 for a short tour and vacation;

2. For this purpose, I have sufficient and adequate financial capacity to support and defray
the said travel and do hereby undertake to fully finance Jann Andreii C. Cruz’s trip
abroad, from the application fees, airline tickets, board and lodging, pocket money and
such other expenses that he may incur in the course of the trip;

3. Jann Andreii C. Cruz would neither be a burden to the state nor at their place of
destination at any time during our trip abroad;

4. There is no criminal case pending in court against him nor has he been charged of
subversion, rebellion, insurrection or any crime or offense involving moral turpitude in
any court or tribunal in the Philippines;

5. I am executing this affidavit to declare the truth of the foregoing facts and for whatever
legal purpose it may serve.
