MONITORING AND EVALUATION PLAN FOR

MEP/ALIVE PROGRAM 2023 PROJECT PROPOSAL


SY 2023-2024

The proposed MEP/ALIVE project activity intended for SY 2023-2024 has to keep an organized monitoring and
evaluation scheme to achieve success in its implementation. To be able to achieve this target, the activity must apply
the five key performance indicators, namely: ACCESS, EQUITY, QUALITY, RESILIENCY, GOVERNANCE
AND WELL-BEING.

Listed below are the Monitoring and Evaluation Scheme to consider in the conduct of “SCHOOL-BASED PROJECT
#GO ALIVE FOR MEP/ALIVE (GOAL-ORIENTED ACTIVE LEARNERS OF INCLUSIVE VITAL
