R/E Ms Margaret Helen Martin 81 years old

Dear Nurse,

I would like to introduce to you, Ms Margaret Helen Martin, she was recovering from angioplasty
and will be needing frequent home visits for monitoring and anticipate her needs following her
return to home tomorrow.

Ms Martin was recovering from her second angioplasty, after being diagnosed of Coronary
Artery disease. Her post operative recovery is remarkably satisfactory but a bruise in the
catheter site was noted.
During confinement she was monitored by a dietician and was told to have low calorie, high
protein, low cholesterol, gluten free diet in small frequencies. She was also encouraged to
perform daily light exercises, monitored by a physiotherapist.

Ms Martin was being treated since 2008 for hypertension, hypercholesterolemia and Coronary
artery disease. In addition she had her first angioplasty in the same year.
She was overweight and had hearing difficulties.

In would be beneficial to Ms Martin if you could arrange home visits to ensure adherence to her
post operative medication exercise and dietary regimen. It would be greatly appreciated if she
would be regularly checked upon by a Doctor, dietician and physiotherapist. Kindly take note
that she is at risk of social isolation due to infrequent family support.
