Mr. O' Riley requires your professional assistance regarding his request for low fat dietary
guidelines and healthy simple food recipes.

Mr. O' Riley was admitted to St Andrew's hospital 6 days ago, and was diagnosed of obstructive
coronary artery disease, subsequently he undergone coronary artery bypass graft 4 times on
September 4, 2009.

Mr. O'Riley was single and lives alone in his residence outside Goondiwindi, He was a smoker
and consumed 20 cigarettes sticks per day, he also frequently drinks two beers a day. Because
of his fondness for fast food and fried foods, Mr O' Riley was overweight, he is also known to
have allergic reactions to nuts.

He was advised to follow up visit with local GP Dr. Avril Jensen at 2pm of September 15, 2009

It would do him good to continue rehabilitation exercises with a local physiotherapist.

It would also be beneficial to provide him a list of low fat dietary guidelines and healthy simple
food recipes, it would be greatly appreciated if you could send this at Mr. O'Riley's home at
