1.0 INTRODUCTION
This project is aimed at providing SURFACE PREPARATION AND PAINTING WORK AT
OKPELLA, OKPELLA, EDO STATE

This Health, Safety and Environment plan is meant to ensure and demonstrate that all
hazards and risks associated with the accomplishment of the SURFACE PREPARATION AND
PAINTING WORK AT OKPELLA, EDO STATE have been identified, assessed, controlled
and reduced to As Low As Reasonably Practicable (ALARP).

NINONENA INTEGRATED SERVICES LTD (NISL) is committed to ensuring the health,


safety and security of all its stakeholders as well as preserving the integrity of the environment
within its area of influence.

We understand that safety is a collective responsibility; therefore all personnel involved in the
execution of this project are fully responsible for the achievement of the project HSE objectives
and targets.

2.0 PURPOSE
The purpose of this HSE Plan is to define the Project’s safety management strategies
for the SURFACE PREPARATION AND PAINTING WORK AT OKPELLA, OKPELLA,
EDO STATE activities to be carried out on the Project, in accordance with the
contractual HSE requirements. This HSE Plan shall provide a framework for
monitoring and overseeing activity compliance with Project safety management
requirements.

3.0 SCOPE
- The HSE Management plan describes:
- The HSE policy for the project,
- The Objectives and Targets,
- The significant HSE issues related to operational and technical aspects and the associated
key risks,
- The procedures and control processes that will be used to mitigate risks and deal with the
consequences, including emergency response preparation and contingency plans,
- The implementation of continuous improvement in HSE performance

5.0 DEFINITIONS AND ABBREVIATIONS

5.1. Definitions

Accident An undesired event that results in physical harm to a person or damage


to property (Refer to Incident).
Audit Means a systematic review of the safety management systems being
applied on the project.
Company Shall mean DCP (COMPANY)
Contractor Shall mean NINONENA INTEGRATED SERVICES LTD.
Continual The process of enhancing the Project safety management system as

improvements in overall safety in line with the NISL HSE Policy
Duty of Care A statutory legal obligation to ensure the health and safety of specified
persons.
Incident Means an undesired event that results in physical harm to a person or
damage to property, or, any “Near Miss” incident that has the potential
to cause physical harm to a person or damage to property.
Inspection Means a review or check on the safety requirements being
implemented.
ROW Means Right-of-Way which includes the easement and/or working
width or additional work space acquired or to be acquired by the
Company for the construction, operation, inspection or repair of the
pipeline.
Subcontractor Shall mean a contractor engaged by NINONENA INTEGRATED
SERVICES LTD.
MSDS Material Safety Data Sheet, documented information which clearly
identifies a hazardous substance and states the precautions to be taken
for the safe storage and use of the substance, the health hazards
associated with the substance, and the results of any tests carried out for
the substance that are relevant to its safe use.
Job Safety A study carried out to ensure that hazards are identified, analysed, and

Personal Protective Means clothing or equipment which, when used correctly, protects part

8.0 NISL HSE ORGANISATIONAL MANAGEMENT STRUCTURE


The HSE Organisational structure for the project is provided in Appendix 1.

8.1. Project Manager


The Project Manager has ultimate responsibility for the overall performance of the HSE
Management System of the entire organisation. He is responsible for the effective planning,
organising and controlling the resources of the company to achieve Health, Safety and
Environmental policies and objectives
The Project Manager is the focal point for issues pertaining to the project and shall interface
with the client representative as required. He shall plan work and ensure that
 activities are carried out in accordance with the HSE requirements of the project
 competent personnel are deployed on this project
 all field team members are aware of the HSE requirements on the project site
 the NISL HSE objectives are consistent with those of DCP-DANGOTE on the project
site
 all tools and equipment used in the project are functional, appropriately calibrated and
have no adverse HSE potential
 incident / accident reports are properly investigated and corrective actions taken
 selected sub-contractors’ activities are monitored and deficiencies in equipment,
standards, operating procedures are corrected

8.2. Quality Assurance and Quality Control Coordinator (QA/QC Coordinator)


The QA/QC Coordinator is responsible for the coordination of all QHSE related activities,
and for:

 Ensuring continual implementation of the QHSE management systems.


 Ensuring that equipment used for work are functional, and have been appropriately
calibrated.
 Monitoring the HSE performance of the organization, through audits, inspections and
relevant key performance indicators
 Receiving and collating accidents/incidents from projects to produce statistical analysis
and proffer recommendations for improving the work practices
 Liaising with client representative on behalf of NISL on HSE issues and relevant
authorities

8.3. Safety Supervisor

The Safety Officer shall:


 Ensure that all employees on the project comply with and observe all NISL and DCP-
DANGOTE HSE policies.

 Promote employee understanding and alertness to security and safety, and associated
importance to both NISL and DCP-DANGOTE operations.
 Ensure that all employees are acquainted with the risks and hazards associated with the
work at Site.
 Arrange HSE meetings
 Organise tool box meetings
 Observe work practices, report anomalies and accidents at Site
 Work with the Project Manager to coordinate community issues
 Be responsible for reporting and initiating emergency response procedure

8.4. First Aider

The First Aider shall:


 Report and follow-up on illness or injury at work.
 Maintain first aid kit and keep record of use.
 Liaise with retainer clinic in emergency situations

8.5. Team Members


The other team members comprise the engineers, technicians, geologists, sub-contractors and
community personnel. These personnel shall
 take reasonable care for the health and safety of themselves and others
 make sure that they wear their PPE’s at all times while at work
 report all unsafe acts and unsafe conditions to the immediate Supervisor or Safety
Officer
 attend tool box and all other HSE meetings on site
 be familiar with any information published by the company or brought to their notice
regarding the safe use of equipment, plant and materials relevant to their work

9.0 PROJECT HSE OBJECTIVES


The Project HSE Objectives:
 Achieve zero fatality,
 Achieve zero lost time incident,
 Achieve zero transport accident,
 Zero security incident,
 Ensure use of safe and functional equipment throughout the duration of project,
 Ensure that only medically fit personnel are engaged in the project,
 Ensure integrity of emergency preparedness and response procedure, and
 Ensure conservation of the work environment.

10.0 HSSE MANAGEMENT ON SITE

NISL is committed to conducting its activities in a manner that the health and safety of its
employees and stakeholders are assured, with due consideration to protection of the
environment through the adherence to relevant policies (Appendix 2) and established
procedures. Consequently,

 All personnel shall have a site safety briefing, including details of the project
 Relevant Work permit shall be obtained prior to the commencement of any job that
requires it.
 All personnel are to report unsafe acts/conditions, all accidents and near misses.
 Appropriate PPE must be worn on the work site at all times
 Personnel shall be reminded of all emergency procedures/contingency plans
 All personnel shall attend the daily Pep talk (toolbox meeting)
 only First aid Cases shall be handled on site, while cases requiring medical treatment by a
Physician shall be handled at the retainer clinic

10.1. Hazard Management Inspection / Audit

Frequent and regular inspection of machinery, plant, equipment, tools, materials, work sites
and other facilities involved on the project will be carried out to eliminate hazards and to
assure that all conditions are in accordance with applicable requirements and standards. NISL
INTEGRATED SERVICES LTD shall take corrective actions on all non-conformances
identified during internal audits, external appraisals, audits and surveillance/inspection
activities. Formal corrective action of the system at fault to prevent similar occurrence shall
be implemented within two weeks.

All corrective actions shall be subject to follow-up audits by NISL INTEGRATED


SERVICES LTD to ensure that corrective action has taken place and is effective. These
follow-up audits shall be conducted not later than 4 weeks after the original corrective action
has been identified.

Each audit or review shall be subjected to a comprehensive written report, a copy of which
shall be available to DCP-DANGOTE, within 5 working days of the last day of the
audit/review.

10.2. Permit-to-Work

NINONENA INTEGRATED SERVICES LTD shall ensure that permit-to-work and


associated specific work type complimentary certificate(s), when required, shall be obtained
and duly signed by the relevant Client Representatives before commencing work on any
aspect of the project.
The permit shall be counter-signed by the NISL staff (the recipient), who will be directly in
charge to supervise the work to be carried out under the permit.
The permit to work shall be revalidated on a daily basis.

A copy of the appropriate PTW shall be maintained at work site and would be made available
as may be required during audit or other purposes. All precautions stated in the PTW shall be
complied with by NINONENA INTEGRATED SERVICES LTD and shall be ticked for
indication.

On completion of the work, permits shall be signed off by the AHSS and Permit Holder.

10.3. Personal Protective Equipment

Prior to commencement of job, the appropriate personal protective equipment (PPE) shall be
distributed to staff. These PPEs shall include safety boots, helmets, hand gloves, coveralls,
safety harness, safety goggles, etc. as appropriate to each task. Use of the PPE is mandatory
while at work.

The following shall be the minimum PPE requirement at site:

(i) Safety boots shall be worn at all times at work site.


(ii) Safety helmets shall be worn whilst working where there is a risk of falling objects
(iii) Fire retardant coveralls shall be worn at work site.

Routine inspection shall be carried out at each site by the Safety Officer to ensure that
employees use and care for their issued PPEs. Non-compliance shall be reported to the Project
Manager for appropriate disciplinary action.

10.4. Emergency Response

NISL HSE Management System covers several emergency situations arising from fire
outbreak and medical cases. Drills are conducted quarterly, but a minimum of one drill will be
carried out at site in the course of the project.

10.4.1. Fire Incident

- In the event that a personnel perceives the smell or sees smoke/fire, such staff should
raise the alarm – shout FIRE! FIRE!! FIRE!!! and walk briskly to the Mustering point.
- Personnel who hear the alarm should assemble at the Mustering Point.
- Report to the Project Manager or Safety Supervisor giving exact location and full details
- If it is safe to fight the fire, do so with a portable fire extinguisher.
- At the mustering point, the Safety Supervisor makes a roll call of all personnel and
organizes search for any missing person(s)

10.5. Incident Reporting, Investigation and Follow-up

Incident/accident reporting procedure is fully implemented in NISL operations. On the


project site, incidents/accidents shall be reported to the DCP-DANGOTE Representative.
Where an accident occurs, the DCP-DANGOTE Representative shall be notified verbally
within 1 hour. A preliminary report shall be submitted within 7 days. A joint Accident
Investigation Team comprising both relevant DCP-DANGOTE and NISL Representatives
shall be constituted to investigate and follow-up accidents.

10.6. Commitment to Health

In order to deal constructively with the health of employees in relation to their work, NISL
shall provide preventive medical care and health maintenance to its personnel.
 Personnel shall be certified medically fit at NISL’s retainership clinic; original medical
certificate shall be maintained at the base Office.
 Personnel shall be required to imbibe the culture of personal cleanliness

 Personnel shall not to indulge in self medication


 Well-equipped first aid box manned by a First Aider shall be available at Project Site
 Personnel shall not be permitted to consume or be in possession of drugs or alcohol
during work

10.7. Journey Management System

NISL ensures that its transportation operations are carried out in such a way as to minimize
exposure of people, assets and the environment to possible hazards. Night driving is not
permitted on this project.

All journeys out of the project site after 18.30hours shall be restricted only to the following
circumstances, as authorized by the Project Manager:
- Operational emergencies
- Medical emergencies
- Security operations (with authorized security escorted movements)

Drivers of NISL vehicles are required have appropriate experience and possess drivers’
license. Such drivers are made to participate in Defensive Drivers Course according to our
internal Training Programme or when such Courses are provided by Clients

10.8. Communication with Workforce

Communication within the project team shall be informal as the need arises and at meetings
(HSE and Tool box).

 Site Instructions/Memorandum
Critical HSE issues on this project shall be communicated to the Project Manager
through memorandum. He will pass information or instruction verbally to all personnel
involved in the project as applicable

 HSE Meetings
HSE meetings will be held weekly. They will be coordinated and documented by the
Safety officer; to communicate the project HSE targets, objectives, improvement plans,
commend compliances and correct non-compliances. Copies of the meeting shall be
maintained.

 Tool Box Meetings


START (Safe Task Assignment and Responsibility Training) shall be carried out every
morning to inform the workforce of the safety requirements of their task and
responsibility. Content of work permit and relevant section of JSA shall be part of daily
discussion.

HSE alerts and publications when received at site shall be communicated to personnel of
immediate danger.

10.9. Risk Assessment

Systematic approach is adopted to identify, analyse, evaluate hazards and risk for the
purpose of establishing controls to reduce such hazards to as low as reasonably
practicable.

The specific Job Hazard Analysis is prepared in line with a risk assessment matrix. It
contains an overview of the potential risks/hazard associated with job activities and
their control or recovery measures.

All personnel shall be informed about these hazards, their control and recovery
measures during pre-mobilisation meeting and subsequent daily toolbox meetings
before commencement of work.

10.10. Waste Management

NISL is committed to the preservation of the integrity of the ecosystem within its area of
influence. Consequently, we shall keep our work area tidy and ensure that there is no
accumulation of waste. Wastes shall be segregated at site and disposed at recommended dump
sites.

10.11. Security
The following steps shall be taken at the Project Site, to ensure the security of staff:
 awareness at tool box meetings
 personal conduct of staff
 compliance with applicable client’s security policies at Site
 personal identification of personnel (display of identity cards)
 authorisation of personnel movement during and after work hours
 personnel are not permitted to carry weapons
 engagement of host community as day and night guards

10.12. Training

In NISL, training is carried out internally or externally by a third party. We also key into
Training Programmes provided by our Clients. However, personnel (including community)
deployed on this project shall pass through an HSE induction where the following HSE topics

10.13. Management of Sub-contractors

Close supervision of subcontractors’ activities is critical to the achievement of NISL’s HSE


objectives.

NISL ensures that:


 Subcontractors are prequalified based on HSE and technical capability,
 Subcontractors comply with NISL’s HSE policies and relevant standards.
 Subcontractor personnel are competent to perform assigned tasks, and where appropriate
hold a certificate of competency.
 Subcontractor and the NISL Project Manager are mutually aware of each other's
obligations for HSE management
 The responsibilities of both the subcontractor and NISL for HSE management are
clearly communicated and defined within the terms and conditions of engagement
 Monitoring of performance of the subcontractor is complimented by Site inspections.

10.14. Community Relations

Project sites are often within a location delineated as client property. NISL representative
shall liaise with DCP-DANGOTE CLO on community relations issues. Nevertheless, NISL
shall engage unskilled personnel from the area. Issues pertaining to community relations shall
be treated before mobilisation to Site

11.0 HSE AUDITS REVIEWS AND INSPECTIONS

11.1. Pre-Execution Audit

Upon contract award, the Project Manager shall invite appropriate DCP-DANGOTE
Representative for a pre-execution audit of NISL’s facilities for the project. The result of this
audit shall correct any concerns.

Corrective actions shall be implemented where deficiencies exist.

Where DCP-DANGOTE is satisfied with the audit, mobilization shall be approved for NISL
to mobilize to Site. Subsequently, after mobilization to site, relevant personnel of NISL shall
accompany DCP-DANGOTE Representatives on site audits.

11.2. HSE Inspection

On a daily basis, hazards on the project shall be identified and documented by the site Safety
officer. Hazard Observation form shall be used in recording all types of anomalies including
unsafe acts/unsafe conditions, nearmisses, environmental anomalies, Security Lapses. The
observed anomalies shall be rectified immediately by the responsible personnel and verified by
the Safety Officer. Reports shall be submitted to the Project Manager and DCP-DANGOTE

APPENDIX 2 - HEALTH, SAFETY AND ENVIRONMENT POLICY

 Has a systematic approach to HSE management designed to ensure compliance with the law and to
achieve continuous performance improvement
 Set targets for improvement and measures, appraisal of all staff and rewards accordingly.
 Requires its sub-contractors to manage HSE in line with this policy
 Includes HSE performance in the appraisal of all staff and rewards accordingly.

It is our policy to provide and maintain a safe working environment for our employees, who are our
most valued assets. The safety of our employee is our utmost priority and all safety equipment must be
provided before any job is carried out.

All staff of NINONENA INTEGRATED SERVICES LTD shall be medically fit. We shall maintain
an insurance against any liability that may result in case of any personnel suffering occupational
disease or any impairment to health resulting from our operation. The company’s retainer clinic shall
handle all routine medical and emergency cases.
As an essential part of the overall company’s management, NINONENA INTEGRATED
SERVICES LTD takes responsibility for security of all personnel, client representative and third party
in all our work sites and premises.

In carrying out our operations, NINONENA INTEGRATED SERVICES LTD shall plan and
execute it in such a manner as to eliminate adverse effects or impact on the environment. Our
operations shall be essentially environmental friendly.

NINONENA INTEGRATED SERVICES LTD shall identify with the needs and aspirations of her
clients and the host communities in the execution of its operation. We shall liaise with clients, host
community, relevant government departments and others to ensure safe and peaceful working
environment. Harmonious relationship shall be maintained with clients and community. Where there
are conflicts, peaceful dialogue shall be employed to resolve areas of disagreement.
