CHECKLIST

New FSP Licence Application

Use this checklist to ensure that you have all the documentation required before applying for an
FSP Licence.

Below is a list of the documents you need to submit to the Financial Services Board (FSB) along
with your completed FSP licence application forms.

Please note:
Application forms and required documentation vary across licence category applications. The
documents listed below relates to a CAT I FSP licence application.

Registration documents for the legal entity (CC or Company from CIPC) or Trust Deed and Letter of Authority
(Trust)

Certified copy of Identity Document (of each KI)

Certified copy of Highest Qualification (recognised Qualification) of each KI

Certified copy of RE certificate(s) (RE1/RE5) of each KI

Short CV of each KI

Reference letter(s) for KIs reflecting management experience and experience in categories and sub-categories
being applied for

Role of KI in the business

Business Plan (i.e. how business will be conducted) / Business Strategy

In the case of a new entity, projected financial statements for a 12 month period is required and confirmation from
auditor or accountant, whichever is applicable, that the entity has not traded.
In the case of company/CC that has been trading, a copy of the latest financial statements is required.

If an accountant, auditor or independent review is required by the entity, a letter confirming:

The appointment to the entity;


If applicable, that no trading has been done by the entity;
The firm and responsible person:
- Is organisationally independent from the FSP, or the group of which it is a part of and able to
maintain an objective frame of mind in accomplishing the responsibilities
- Keeps up to date with developments in the profession and within the financial statements industry;
- Has sufficient and relevant knowledge of the industry for the engagement;
- Is qualified to act for the FSP
- Has access to a library with up-to-date sources of relevant statements, standards, legislation,
regulation, literature, trends, and developments within the financial services industry.

If Long Term Insurance: sub-category A (sub-category 1.1) is selected, provide the names(s) of Assistance
Business FSPs and/or Long Tern Insurance companies with whom the business has agreements.

If applying for Health Service Benefits, the Accreditation Certificate for both the entity (if applicable) and each Key
Individual to be appointed for this product category, is required.

Proof of payment of the application fee
