1. This investigation was conducted pursuant to the institution of chief of police of Las Navas
Police Station, Christian Aurel, about the Vehicle incident of no. 097783533674 of September 1, 2023

4. To look into the facts and circumstances our team has been working delight and analysed the
incident scene. We interview witnesses and obtain CCTV footage in the nearly establishments related
to the incident, we collect marks and even photograph the evidence.

7. On September 1, 2023. Prior to the investigation both victim are unconscious the SUV
(vehicle B) and Sedan (vehicle A) after the big impact of collision, because of not properly fallowing of
traffic light in the road leads to collision by sub vehicle B. While sub vehicle B preparing to stop it’s too
late to break the vehicle because in that time he was sleepy while driving, and notice that sedan vehicle
was in front of him.

lV. Discussion and Findings

8. An assessment of the accident scene suggests a high-speed collision resulting in significant

damage to both Vehicle A and Vehicle B. both drivers have been identified and interview and gather
their accounts of the incident. According to the driver of Sub Vehicle B, He was from the Duty in Night
shift that’s why he was sleepy was driving the vehicle and admit that he caused the accident in the Road,
,while driver of sedan vehicle is still unconscious cause by the accident. CCTV footage also showed the
sub vehicle has collided to sedan vehicle.

V. Conclusion

9. After careful perusal of circumstances surrounding this particular incident, it was sub vehicle
B in the Inter action of H. Jole-jole Quirino Street that caused the incident who leads Serious Physical
Injury and Reckless Imprudence.

10. Therefore in view of the foregoing this office hereby recommends that a case of Reckless
Imprudence leads Serious Physical Injury with the punishment of 4 months and 1 day to 2 years and 4
months or a fine raging from P20,000 to P100,1000 if it is too serious Nature. Be filed against sub vehicle
B. the owners name is Itan Au. Male 30 years of Age resident of Pob 1. Catubig N Samar Philippines
