EXTERNAL EXAMINATION
Annex “B”

A disruption in the normal function of the brain that causes by bumping


her head on an object that pierces the skull and enters brain tissue.

There is heavy charring of the entire head and neck and the scalp over
the calvaria is completely burned off.

The calvaria has a displaced fracture of the skull extending from the
right brow down to the left inferior occipital area.

The genitalia are that of an adult female and there is evidence of


injury. Parietal, temporal bones and skull are not equal, symmetrically
developed and show evidence of injury.

INTERNAL EXAMINATION:

HEAD—CENTRAL NERVOUS SYSTEM: The scalp is charred throughout. There is an


open displaced fracture of the left frontal bone in the forehead area.
Opening the skull reveals thin subarachnoid hemorrhage over the base of
the brain, with intensive laceration of the base of the brain.

SKELETAL SYSTEM: There is an open fracture of the forehead, a displaced


ring-type basal skull fracture, post-mortem thermal fractures of the
right humerus, right pelvis, and right femur.

RESPIRATORY SYSTEM: The lungs are well inflated and light pale fluffy.
The visceral pleura is intact, smooth and without adhesions.

CARDIOVASCULAR SYSTEM: The heart weighs 255 grams, and has a normal size
and configuration. No evidence of atherosclerosis is present.

GASTROINTESTINAL SYSTEM: The esophagus has extensive periesopheal


hemorrhage (traumatic hemommediastinum). The stomach is intact and
contains 10 cc of pale tan fluid (not collected). The small intestine is
pale and anemic as it is the large intestine. They have normal light-
colored content. The appendix is present. They mesentery has no tears or
hemorrhage.

URINARY SYSTEM: The kidneys are smooth and vary pale pink anemic, and
occupy a normal position in the retroperitoneum, the right kidney
weighing 150 grams and the left kidney 150 grams. They have no injury.

FEMALE GENITAL SYSTEM: The structures are within normal limits.

TOXICOLOGY: Sample of right pleural blood and bile are submitted for
toxicologic analysis. Stomach contents are saved.

SEROLOGY: A sample of right pleural blood is submitted in the EDTA tube.


Routine toxicologic studies were ordered.
LABORATORY DATA

Cerebrospinal fluid culture and sensitivity:


Gram stain: Unremarkable
Culture: No growth after 72 hours
Cerebrospinal fluid bacterial antigens:
Hemophilus influenza B: Negative
Streptococcus pneumoniae: Negative
N. Meningitidis: Negative
Neiserria meningitidis B/E. Coli K1: Negative

Drug Screen Results:


Urine screen {Immunoassay} was NEGATIVE.
Ethanol: 0 gm/dl, Blood (Heart) Ethanol:
0 gm/dl, Vitreous

EVIDENCE COLLECTED:
1. One (1) Pink t-shirt, size large.
2. One (1) pair blue jeans, size large.
3. Samples of Blood (type O+), Bile, and Tissue (heart, lung, brain,
kidney, liver, spleen).
4. Twelve (12) swabs from various body locations, to be tested for
presence of hypochlorite.
5. One postmortem CT scan.
6. One postmortem MRI.
OPINION
Time of Death: Body temperature, rigor and livor mortis, and stomach
contents approximate the time of death between 6:40: and 7:20 P.M. on
09/25/21. Immediate Cause of Death: It is a form of traumatic brain injury
in which there is heavy charring of the entire head and neck and the scalp
over the calvaria is completely burned off. The calvaria has a displaced
fracture of the skull extending from the right brow down to the left
inferior occipital area It happens when the brain rapidly shifts inside
the skull as an injury is occurring.

Manner of Death: Homicide


Remarks: Decedent originally presented to this office as Reckless
Imprudence Resulting of Homicide victim.
Cebu City.
Investigators were notified of this finding immediately upon conclusion of
examination.

FACTS OF THE CASE:

That on or about 1st day of September 2022 at about 08:00 o’clock in the
evening in Juan Luna Avenue Extension, Cebu City, Philippines and within the
jurisdiction of this Honorable Court, the above-named accused, with in
voluntarily but failing to do an act which cause damage and harmful effects, did
then and there willfully, unlawfully and feloniously, Crashed a motorcycle owned
by Bianca Winsley due to taking an overtake but did not foresee the motorcycle of
the victim which resulted to his instantaneous death, to the damage and prejudice
of the family of the victim to be proven during the trial of the case.
