Re: Delayed Registration of Marriage

We, JEREMIAH M. TORDA and HERNIÑA KRISTEL S. BERNARDO,

Filipinos, both of legal age and residents of No. 406 P. Bernal Street, Ugong,
Pasig, under oath, do hereby depose and state, that:

We contracted marriage on March 17, 2023 at Queen’s Garden Greenland

Subdivision, Cainta, Rizal, and was solemnized by Eugenio Leonardo C. Santos,
Officiating Minister, Christ’s Commission Fellowship;

Unfortunately, however, due to conflict of schedule, we are unable to

timely process the registration of our marriage before the Local Civil Registrar of
Cainta, Rizal;

This affidavit is being executed to attest to the veracity of the foregoing

statement in order to prove the facts of our marriage for purposes of the delayed
registration of marriage and for purposes not contrary to law, morals, good
custom and public policy.

IN WITNESS WHEREOF, We have hereunto set our hands, respectively,

this 24th day of November 2023 in Cainta, Rizal, Philippines.

state, that:

I solemnized the marriage of spouses Jeremiah M. Torda and Herniña

Kristel S. Bernardo on March 17, 2023 at Queen’s Garden Greenland
Subdivision, Cainta, Rizal;

Unfortunately, however, due to conflict of schedule, the said spouses were

unable to timely process the registration of their marriage before the Local Civil
Registrar of Cainta, Rizal;

This affidavit is being executed to attest to the veracity of the foregoing

statements in order to establish the fact that I solemnized the marriage of the
aforesaid spouses, for purposes not contrary to law, morals, good custom and
public policy.
