AFFIDAVIT OF BEING SINGLE

1. That I am applying for enlistment in the Armed Forces of

the Philippines;

2. That I am still single as I have never contracted any

marriage with another person up to present and have not
sired any child;

3. That I am executing this affidavit in good faith to

attest the fact of being single/unmarried and for
whatever legal purposes this may serve best.
