SUPPLEMENTAL COMPLAINT-AFFIDAVIT

I, RECHELLE A. REVILLOZA, Filipino, of legal age, single and with postal

address at Block 2 Lot 21 Shooting Star Street Antipolo Hills Subdivision, Antipolo City,

Rizal, after having been duly sworn to in accordance with law, states: That --

1. Further to my Complaint-Affidavit dated __ 2008, I hereby submit

Certification dated 06 December 2006 from the Philippine Overseas Employment

Agency, Licensing and Regulation Office, hereto attached as Annex “A” to form an

integral part hereof.

2. In said certification, it can be readily perused that respondents’ agency,

EUROPEAN TRAVEL AND TOURS, with reported address at 189 Mhearle Tale

Mansion, Barangay Toro, Quezon City, is not registered as a legitimate recruitment

agency. As such, respondents are clearly guilty of illegal recruitment, not to mention

misappropriation and estafa;

3. I am executing this Supplemental Complaint-Affidavit to attest to the truth

and veracity of the foregoing facts and for the purpose of charging MARITESS

BATAC, MARLENE MILUCHA and ORLANDO DELA CRUZ with FIVE (5)

COUNTS OF ESTAFA as defined and penalized under Article 315, paragraph 2(a)

of the Revised Penal Code, as amended and ONE (1) COUNT OF VIOLATION OF

SECTIONS 6 and 7 of R.A. 8042, otherwise known as the “Migrant Workers and

Overseas Filipinos Act of 1995”.
