AFFIDAVIT OF CLOSURE/ CESSATION OF BUSINESS

I, RYAN CORDERO EDIOS, of legal age, Filipino, married and a

resident of Blk 4 Lot 3 Country Homes, Mankilam, Tagum City, Davao del
Norte, Philippines, after having been duly sworn to in accordance with law,
do hereby depose and say:

1. That I am the bona fide owner of a business involving service

activities related to printing, retail of books, office/ school
supplies, newspapers and magazines and other computer
related activities with a trade name: QUICKLINE
ENTERPRISES, situated at Stall #10, Temp Linear, Crossing,
Poblacion, Compostela, Davao de Oro 8109;

2. That due to the impact of the pandemic (COVID 19) the

business operation and income was greatly affected and thus I
am left with no other recourse but to cease doing business in
the said area;

3. That I am executing this affidavit to attest the truth of the

foregoing facts, to inform the proper authorities concerned,
especially the Office of the Municipal Mayor Business
Permit and License Division, Bureau of Internal
Revenue, Department of Trade and Industry and other
related government offices regarding this matter to expedite
the cessation of the said business.
