It also evaluates a care plan students prepared, worth 30 points total - assessing the number and type of nursing diagnoses, ability to make a plan from collected data, preparation format, and priority planning. Finally, it evaluates laboratory skills out of 20 points and case study discussions out of 30 points. The form is used to assess students during their practice placement in these areas.
