WAIVER FOR NON-SUBMISSION OF BIR FORM 2316

I, KIM JOHN ANACIO LANDICHO, Filipino, of legal age, and with address
2460 Patria St. Balut Tondo Manila, who by virtue of this hereby confirms and affirms
that;
1. I was hired by Ubiquity Global Services, Inc. “The Company” as IT Support
Associate on December 15,2023;
2. As part of the Company’s pre-employment requirements, I was required to
submit BIR form 2316 (Employer’s Certificate of Compensation Payment/Tax
Withheld for Compensation Payment with or without Tax Withheld) from FB
Management Group Inc. for the employment I rendered to them for February
8,2023-October 15,2023;
3. The Company has exercised due diligence to gather my BIR form 2316 but I have
failed to submit said document due to I did not furnish my current employer
Ubiquity Global Services, Inc., a copy of 2022 Certificate of Income Tax Withheld
on Compensation (BIR FORM 2316) from my previous employer FB
Management Group Inc.;
4. In lieu of the BIR Form 2316, I am issuing this waiver exonerating and holding
the Company free and harmless from any and all obligations arising from the
limited information that the Company has with regard to my income;
5. I am holding myself responsible for filing my consolidated annual return for
2022 and settling whatever deficiency withholding taxes that may due or that
later on may be determined by the tax authorities; and
6. I am voluntarily issuing this waiver bearing in mind its implications and legal
repercussions.
