Dear Ms. Peñarubia:

This serves as confirmation of your probationary employment with PKI Manufacturing


and Technology, Inc. as Coordinator/Safety Officer effective June 16, 2020.

Under this contract, you accept the following terms and conditions:

1. As Coordinator/Safety Officer you will receive the following renumeration payable


every 15th and 30th of the month:

2. Your work schedule shall be from Monday to Saturday, 0800H – 1700H

3. You will report and be directly responsible to the Department Manager with the
supervision of Mr. Ernesto Belen Jr. who shall clarify your duties and
responsibilities.

As Coordinator, you shall be responsible in the following duties and


responsibilities:
 Perform other duties assigned by the Operations Manager.
 Responsible in coordinating workers concern and inquiry and relay to the
HR Department for immediate action;
 Assist the workers staff in terms of results by communicating job
expectations; planning, monitoring, coaching, counselling, and disciplining
employees; initiating, coordinating, and enforcing systems, policies, and
procedures.
 Maintains work flow by monitoring steps of the process; setting processing
variables; observing control points and equipment; monitoring personnel
and resources; studying methods; implementing cost reductions;
developing and reporting procedures and systems; facilitating corrections
to malfunctions within process control points; initiating and fostering a
spirit of cooperation within and between departments.
 Assist the HR Department in collection of workers daily time attendance
for purposes of computing the payroll;
 Assist the Administration department in disseminating ATM cards of newly
hired employees;
 Maintains quality service by establishing and enforcing organization
standards.
 Responsible for the quality of work in compliance with ISO 9001:2008
Standard.

As Safety Officer, you shall be responsible in the following duties and


responsibilities:
o Support the development of OHS policies and programs
o Initiate and organize OHS training of employees and executives
o Review existing policies and measures and update according to legislation

o Conduct orientation to newly hired employees prior to deployment on


various safety-related topics (noise levels, use of machinery etc.)
o Inspect premises and the work of personnel deployed to various clients to
identify issues or non-conformity (e.g. not using protective equipment);
o Conduct risk assessment and enforce preventative measures
o Stop any unsafe acts or processes that seem dangerous or unhealthy
o Record and investigate incidents to determine causes and handle worker’s
compensation claims
o Prepare reports on occurrences and provide statistical information to upper
management

4. Any confidential information that you may have access to by virtue of your
position, including your remuneration, are confidential matters and should not be
disclosed to others.

PKIMT may terminate the employment contract of the Employee at any time:
 If the business is terminated owing to the circumstances beyond the employer’s
control and the employer can no longer utilize the employee’s services;
 If the employee commits any criminal act;
 If the employee is absent from work from her/his duties for a period of three (3)
consecutive days without advance notice;
 For just cause at common law, in which case the Employee is entitled to any
advance notice of termination or compensation in lieu of notice. Without just
cause, in which case PKIMT shall provide the Employee one (1) month advance
notice of termination.
 The employee may terminate her/his employment at any time by providing
PKIMT with at least one (1) month advance notice of her intention to resign.

The employee certifies to the employer that she has read the foregoing terms and
conditions and fully understands the contents thereof, and further certifies that the
foregoing terms and conditions constitute entire agreement with the employer.
