SWORN STATEMENT OF UNDERTAKING

I, ROCKY B. GATCHALIAN, Of ROCKY B. GATCHALIAN with primary

business address at Purok 1 sitio Bugo Kinawitnon District Island Garden City of
Samal, do hereby depose and say that:

1. ROCKY B. GATCHALIAN is a tourism Enterprise as defined under Republic

Act No. 9593 or the Tourism Act of 2009.

2. ROCKY B. GATCHALIAN has submitted its application for a Department of

Tourism (DOT) Accreditation:

3. ROCKY B. GATCHALIAN undertakes to comply with all the requirements for

a DOT Accreditation as well as other DOT standards, rules. and regulations
relevant to Tourism Enterprises:

with all the requirements for a DOT Accreditation, or failure to observe DOT
standards, rules, and regulations relevant to Tourism Enterprises, may result
in the revocation of its Provisional Accreditations, or in its failure to be
granted an Accreditation.
