I understand that Xiklab Group shall conduct background inquiries about me. Should any
discrepancy in my record resulting from falsification or otherwise committed inadvertently may result in
the disqualification of my employment application.

I unreservedly authorize any individual, company, firm, corporation, or public/government agency

to provide all information on me to Xiklab Group or its authorized representative(s) for screening my
employment application. I understand that this authorization or the background inquiries shall not, in
any way, guarantee employment.

I, likewise, release Xiklab Group and its representatives, including any individual or entity who
shall provide information on me, from all liability for damages of whatever kind, which may, at any time,
result to me, my heirs, family, or associates in compliance with this authorization.
