1. PURPOSE
1.1 General issues

This document is to present the Method Statement (MS) xxxxxxxxx prepared by


the Al Kahalejia Co. Kahromika.

Al Kahlejia Co. Kahromika have Quality Management System Certificate


of ISO 9001: 2008 and Certificate of OSHAS 18001: 2007 on Occupational
Health and Safety Management System.

1.2 Purpose

Purpose of this document is to provide instruction for the Dismantling of OHL


pole dismantling. This method statement will be in compliance with the
contract requirements and specification.

2. Scope of Works:

Scope of this MS is to define the:

2.1 Revision of Method Statement Documents

The revision of Method Statement documents shall be done when:


1. New information becomes available.
2. The Project Director requires modifications of Method Statement.
3. There is a significant change in the works methodology.

3.2 Permits and Licenses

The Contractor will ensure compliance with all legal requirements with
regards to permits and licenses from the authorities prior to start of any
activities onsite.

4. ROLE AND RESPONSIBILITIES


4.1 Project Manager

The Project Manager is responsible for:


 Delivering the overall works in a safe and timely manner, ensuring
conformance with the approved design.
 Leading the team of engineers and surveyors, guiding them in the
delivery of the works.
 Verifying that work done is in accordance with requirements of contract.
 Ensuring the quality standards set for the work are achieved and the work
team adhere to the QC and HSE requirements of the contract.

4.2 Project Engineer:

The Project Engineer will be responsible for timely completion of the


works in accordance with KM requirements. He shall be responsible for
the preparation of the works, checking the progress and productivity on
the daily basis and coordinate KM.

4.3 Supervisor:

The Supervisor shall be responsible for the direct execution of works according
to the plan and shall directly control and give instruction to the working
personnel and operator. He shall also ensure that works are performed with
proper tools and equipment and maintain good housekeeping around the
working area. He shall also act as safety officer in the absence of safety
engineer. Responsible for Quality assurance as per KM Standard.

4.4 QA/QC Engineer:

The QA/QC Engineer shall confirm that all site works commenced as per KM
specification and requirements and to submit all Quality forms requested by KM.

4.5 Document Controller

Document Controller is responsible for:


 Documenting, distributing and maintaining data in prescribed format
 Making the necessary data available as requested by the team or the
client
4.6 HSE Engineer:

The Safety Engineer shall submit prior to the commencement of work his
safety plan. He will be responsible for the implementation of all safety related
activities including inspection of safety facilities, conduct training of site
personnel and hold safety meetings periodically. He is responsible to do the
safety audit from time to time and to compile reports.
Note:

Project Manager, Project Engineer, Safety Engineer and Supervisor shall be


fully aware about the HSE policy of KM and concerned organization as related
to the Project.

6. SURVEY CONTROL
Done as per KM Specification.
Surveying to be done KM approved survivor.

7. QUALITY ASSURANCE/ QUALITY CONTROL

The concerned Site Engineer or General Foreman will be responsible to ensure


compliance for these operations and the site QC Engineer will carry out quality
control checks and report the inspection results.

8. IMPLEMENTATION

The following section is split into stages:


1. Preparation and Safety Review
2. Sequence of Works.

8.1.1 Site Preparation

 The area of work has been defined and made safe for the workforce and the
public.
 Buried services and utilities on or adjacent to the worksite have been located.
 Dewatering systems are in place and operational, if required.
 All equipment is in sound working order and has the appropriate certificates.
This is checked regularly throughout the project and when new equipment is
brought onto the site.
 Provision for daily housekeeping will be provided.
 Waste management and disposal systems confirmed to be operational and
effective.

8.1.2 Site Personnel & Communications

 All personnel involved in the implementation of this Method Statement are


competent and have been trained to the required level necessary to be able to
safely execute the activities listed. Only competent and trained personnel will be
appointed for the work.
 All personnel permitted on site (inside and outside of the hoarding area) have
completed a Safety Induction and a Quality Induction. These are mandatory for
all personnel that will be required to work on site in any capacity.
 Vehicle and plant operators are confirmed to hold valid Qatar driving licenses and
3rd party training certificates to carry out their designated tasks. This is repeated
when new personnel come onto the site.

 Smoking will only be allowed in designated areas.


 Daily Tool Box Talks and (pre shift) task briefing complete
 Method Statement briefing, including a signed-off briefing sheet has been
completed (a copy to be maintained for inspection on site).

8.1.3 Maintenance during Site Operations

The following shall be done regularly (time intervals as specified for the relevant tasks)
throughout the execution of the works:
 All equipment inspections shall be updated in accordance with the regulations,
with emphasis on maintenance records and similar for all mobile plant.
 The equipment used will be visually checked and recorded on a daily basis.

8.1.4 Access and Egress

Only authorized personnel will be permitted to enter the work site. Upon arrival at site
the following checks will be made and any non-compliance will result in that person
being prevented from entering the work site:
 Evidence of receiving site specific safety induction (register of attendance /
helmet sticker)
 Appropriate Personal Protective Equipment (see PPE Section).

The following aspects will be ensured as a minimum:


 Access and egress routes shall be clearly defined by designated walkways, traffic
routes, loading and un-loading areas, signage and barriers & lighting (if
applicable).
 Jersey barriers, traffic cones, signage will be used to designate walkways and
traffic routes
 Access and egress routes will not be blocked as a result of the work
 Vehicle traffic routes will be maintained as two way streets
 Plant and machine operators will have all round 360o visibility at all times.
 Regular maintenance of plant will be carried out as per manufacturer’s
recommendation
 Plant movement and operation will be coordinated and managed to ensure safe
and efficient operation.
 Use of mobile phones is not allowed whilst driving or operating plant and
machinery
 All persons (staff & workforce) on site will be briefed on the hazards of working
adjacent to or in the vicinity of moving plant and equipment.
 No sleeping or resting will be allowed underneath stationary plant and equipment
or vehicles.

 No walking, standing or passing under machinery including but not limited to


wheeled loader buckets and excavator arms.
 Flagmen will be used for controlling of traffic.
 No lone working on live carriageways.
 Trench protecting will be done by indicating the public using sign boards and
barriers will be used around the trench area to prevent access during the non-
working hours.

At the end of each working day, all access to the work site will be secured; where by the
Project or Site Engineer will inspect to ensure that:
 There are NO holes in the hoarding.
 Proper housekeeping has been implemented.
 There are NO Hindrances on Access/Egress Routes.

They will hand over the site to the night patrol, and ensure that the work area is secure
and that safety precautions are in place.

When Works are to be executed at night or in times of poor lighting:


 Set out location of lighting towers determined together with HSE Manager.
 Install lighting towers before start of shift or in poor lighting conditions.
 HSE Manager and Project or Site Engineer shall inspect the performance of the
lighting equipment to ensure adequacy of the lighting and improve if necessary.

8.1.5 Emergency Preparedness

An Emergency Response Plan has been implemented at the work site, addressing, but
not being limited to the following issues:
 All accidents and incidents will be reported immediately to site management.
 In the event of a serious accident, the area will be segregated off and a full
investigation carried out with initial report submitted to the client.
 KAH personnel, when necessary, shall meet the emergency services at the site
entrance and escort them to the scene of the accident.
 Emergency contact names and mobile numbers will be displayed in the site
office, entrance gate (security) and rest areas.
 Mobile Plant and Equipment will have fire extinguishers located on each
individual piece of plant.
 Trained first aiders will be available on site and first aid kits available in the site
offices.

8.2 Sequence of Works

 Obtain Permit To Work.

 Close the permit to work.

9. HSE Requirements:
9.1 Objective:

M/s Al Khalejia Co. Kahromika main objective in this program is to ensure the
safety of workers and public at all times. To meet this goal we are going to
implement the following:

a. A safe working environment


b. Safe system of work
c. Continuous dissemination of complete and adequate safety information
d. Training of employees
e. All the execution of works like excavation, road diversion and road
closure shall commence after securing permit from concerned authorities.
f. To properly coordinate with other contractor who have activities in the
same area.

g. The jobsite will be secured by safety fence with safety warning sign
boards at the perimeter and sufficient lighting facilities at night.
h. A safety access for the launch and reception pits will be available by
providing safety ladders securely bolted to the wall and man basket for
egress.
i. To implement policy that only competent and licensed equipment
operators will be allowed to operate equipment and execute excavation
works.
j. Lifting equipment and gears like cranes, slings and shackle shall comply
with Qatar construction standards.
k. Electrical works shall be handled by a qualified person.
All workers must be responsible for their own safety by using necessary safety
equipment and personal protective equipment (PPE), provided by the company such as
safety helmet, safety shoes, reflectorized traffic vest, safety glass, safety gloves and full
body harness if necessary.

9.2 Personal Protective Equipment:

1. All workers must be responsible for their own safety by using necessary
safety equipment and personal protective equipment, (PPE) provided by the
company such as safety helmet, safety shoes, high visibility vest, safety
glass, safety gloves, head lamp and full body harness if necessary.
2. All individual PPE will be monitored by Site Foreman or Safety Officer in
charge of the job site.

9.3 Personnel Welfare:

1. Provide suitable shelter for shade protection during hot weather condition.
2. An adequate supply of fresh drinking water will be provided.
3. Toilets and hand washing will be provided on site.

9.4 Medical / First Aid Facilities

 Prior to the commencement of work, confirmation will be made that all


appropriate and required first aid facilities and trained first aiders are
available on the site or in the adjacent site of PSH. When required, they shall
be available for prompt attention to injured persons.
 The contractor’s vehicle assigned to the job site will be used for transporting
injured persons with minor injuries to the first-aid station or hospital if so

required. In the case of a more serious injury, an ambulance will be used. Any
person injured outside the hoarded area will be immediately transported to
the nearest first aid facility aid.
