Greetings of peace and prosperity!

I am JAYPEE A. BALALANG, 30 years old, designated as Teacher-in-Charge of Nagbabalayan Elementary

School last June 6, 2023 under Office Memorandum No. 099 s. 2023. Relative to my designation, I wish to
signal my intent to be a full-time Teacher-in-Charge of Nagbabalayan Elementary School, Kabugao, Apayao,
for the following reasons;
1. The TIC, to fully perform all duties and responsibilities as defined in RA 9155 and such other related
functions as may be assigned, and
2. Presence of 2 excess teachers in the school.

This school where I intend to be a full-time teacher -in – Charge will have 5 teachers to handle a total of 57
enrollees from kindergarten to Grade VI for School Year 2023-2024.

If granted, hereunder is the school personnel assignment that will be undertaken. (please also refer to the SF7
attached to this Letter for the total actual teaching service per week)
