AFFIDAVIT OF LOSS

I, MARYCEL VENTURA GARCIA, of legal age, Filipino citizen, and a

resident of Birbira, Camiling Tarlac, after having been duly sworn to in accordance
with law do hereby depose and states THAT:

1. I am the Atty-In-Fact of the registered owners of a parcel of land, situated at

situated in Caanamongan, Sta. Ignacia, Tarlac containing an area of
Twelve Thousand Two Hundred Eighty Five (12,285) square meters more or
less, copy of Special Power of Attorney is hereto attached for reference”

2. That the registered owners turned over the owner’s duplicate copy of TCT
No. 043-2019004720, who in turn, entrusted to me the said owner’s
duplicate copy to facilitate so they can have separate titles.

3. One of the documentary requirements for the titling of the Subject Property
is the Owner’s Duplicate Copy of TCT No. 043-2019004720;

4. I, then placed the Owner’s Duplicate Copy of TCT No. 043-2019004720

inside a separate long brown envelope which was in my possession while
transacting with the concerned government agencies, to secure certain
documents being required for the titling of the Subject Property;

5. Based on my recollection, I may have left the said envelope either at the
Registry of Deeds of the Province of Tarlac, at the Municipal Assessors of
Sta. Ignacia, Tarlac and then I returned to the said agencies/offices to check
on the envelope, but the same could no longer be found;

6. When I returned home, I also checked the existence of the said owner’s
duplicate copy of TCT No. 043-2019004720 from my files, but still the
same could not be located;

7. Indeed, I have exerted efforts to locate the same but all proved futile;

8. The above-stated title was never transferred, pledged or used as a security for any
transaction or obligation;

9. I am executing this affidavit to attest to the truth of the foregoing statements

and for all legal intents and purposes it may well serve.
