capacity to work.
To complete this review, you need to provide some information to us before 11/12/2023 or your Supported
Living Payment will stop.
Please provide one of the following that has been completed recently:
l A Work and Income Work Capacity medical certificate from your doctor.
l Medical reports from your specialist(s).
l An assessment from a Needs Assessment Service Co-ordination (NASC) service.
If you can't provide any of this information, or you want to see one of our designated doctors, please let us
know.
When you're getting Supported Living Payment, Work and Income can help you prepare for work, find a job
or do some study.
