Apparently healthy, and
Do not have ticks.

2) The dog(s) (check applicable option):

☐ Have been treated with worm medicine containing praziquantel at a dose of 5 mg/kg
body weight with evidence or documentation provided, or

☐ Are returning to Tasmania after no more than 14 days in another State or Territory
with documentation provided, or

Are exempt from treatment requirement because they are (check one):

☐ Greyhounds racing within 6 days (documentation provided), or


☐ Puppies under the age of 12 weeks, or
☐ Accredited assistance or law enforcement dogs.
