CLIENT PRESENTATION:

Client attended assessment on time with his sister.

MEDICAL INFORMATION:
Client states that he had a suicide attempt on 4 July 2007. Currently client is under the care of the Mental Health
Team at Bankstown and attends a couple of times a week. Client states that he is currently on 2 types of medication.

EDUCATION:
Completed year 10. Continued with TAFE course in CERT III AIN.

EMPLOYMENT HISTORY:
Client reports a limited employment history. Client also states that he has completed his certificate III in Assistant in
Nursing and would like to gain employment within the health care system.

CLIENT GOALS:
AIN employment

ASSESSORS RECOMMENDATIONS AND REFERRAL:

Client would benefit from a referral to JPET provider in his area. Client has a temporary work capacity of 0-7 hours as
per medical certificate. Following this client can return to work at 30+ hours.
