pen (Blue/Black).
2 The application form should be completed and no column should be left blank.
3 Write “Not applicable” where no infromation has to be given.
4 the application form must be accompanied by bank draft of the prescribed amout, in favour of “M.B. Veterinary
College”, payble at DUNGARPUR.
5 The application may be rejected by the college administration at his own discretion without assigning any rea-
son.
6 Self attested photocopies of the following certificates.
a. Four latest colored passport size photographs.
b. Registration form.
c. Admit card of NEET- 2023.
d. Mark sheet & Certificate of Senior Secondary Examination (10+2) ir equivalent recognized exami nation.
e. Mark sheet of 10+1 (11th) having Physics, Chemistry & Biology.
f. Mark sheet & Certificate of Secondary (10th) or equivalent recognized exmination.
g. Character Certificate issused from school/college last attended.
h. Transfer Certificate (T.C.) of last institution.
i. Migration Certificate of last University.or Board
j. Domicile Certificate of respective State issued by competent authority in proper proforma.
k. Cast Certificate issued by competent authority, if applicable.
