DEVELOPMENT OF TECHNIQUE
TECHNICAL CONSIDERATIONS

Microdent developed bone expanders have meant a very substantial novel technological contribution
to oral implantation. The principles governing the use of other classical techniques have been altered
by them: their main basic feature is a tapering screw controlling and retaining the exerted pressure
throughout while maintaining total bone content. Their use is thus practically trauma-free and this
tapering screw bone expansion method can be applied to other fields of medical science.

Main clinical features


All that is needed to start the process is a fissure, or rounded drill.
Bone type D-2 no longer requires processing with intermediate countersinking drills and tapering
tapping.
In bone type D-3, we use excess blood supplied by the bone. The use of only one opening drill cools
osteotomy.
Excellent control of the screw hand or transport osteotome reduces to a minimum risks of lateral per-
foration in the upper maxilla compact bone lip area.
Throughout the process we take advantage of the osseous remains compacted in the neo-alveolus
due to the design of the osteotome screw.
In type D-4 bone, which presents the least percentage of bone tissue of all bone types, we achieve tri-
cortical fastening. Being able to use implants of greater diameter and start from a cortical crest of 1 to
2 mm, allows us to achieve insertions of 5 mm implants.
Finally, our No.6 expander with its concave design permits non-traumatic sinus elevation without
fracture of the maxilla.

Surgical Technique Specially indicated for:


nº1: short starting, 1mm up to 2.30mm Ø tip Thin crests irrespective of bone type.

Advantages
Expands thin crests by increasing width without loss of bone.
Compacts the bone around the implant favouring osteo-integration.
During the operation it gives us perfect control of implant insertion stem.
Inexpensive, as in some cases it replaces the use of drills and prevents them wearing.
Progressive insertion produces bleeding and favours osteo-integration.

When the desired expansion has been achieved, the expander is kept in and, if it is considered advisable, the bed to receive the implant is improved by
drilling at the least possible diameter to ensure the necessary alveolar bed pressure on the implant.

The same principle is followed when placing the remaining implants.
