AFFIDAVIT

I, ROLANDO PRESENTACION BRITO, of legal age, married, Filipino

citizen and with residential address at Brgy. Sapang Palay, Pitogo, Quezon, after
having been sworn to in accordance with law, hereby depose and state; That:

I was born on September 18, 1960 at Tabaco, Albay, Philippines to my

biological parents Fabian Begito Brito and Josefa Cabria Presentacion;

I erroneously used the Middle Name “BIEN” in all of my scholastic

records and other business transactions because I was told by my relatives that
this is the real Surname of my Biological Mother;

Also, I erroneously used AUGUST 06, 1960 as my date of birth in all of my

scholastic records and other business transactions;

When I registered myself as a voter the COMELEC, I used the Middle

Name BIEN and August 06, 1960 as my date of birth. Subsequently, when the
COMELEC issued my Voter’s Identification Card, the Middle Name appearing
therein was “BIEN” while the date of Birth was on “August 06, 2023” and this is
the same Identification Card I used in registering myself in GCASH APP;

Just recently, I found out that the Middle Name appearing in my

Certificate of Live Birth issued by the Philippine Statistics Authority is
“PRESENTACION” and not “BIEN”;

I also found out that my correct date of birth is on September 18, 1960 as
per my Certificate of Live Birth issued by the Philippine Statistics Authority;

I executed this affidavit to attest to the truth of the foregoing facts and to
enable myself to replace my Voter’s ID in my GCASH APP with another
Identification Card bearing my accurate personal information n.
