- Ensure all preparation of work tools and other supporting tools (work access)

- Tools : Machine weld,Machine Orbital, Machine grinding, Machine drill, Measuring

tape, Ring spanner, Slit/Cross screwdriver, Calipers, hand gloves, etc. (All tools used
already checked from at working area).

Job Description :

1. Dismantling Mechanical, Pipe and Electrical Instruments as :

a) . Make sure all control valves CA,DPG, are closed (LOTO)

b) . Make sure all panel at FMT 1-4 position OFF (LOTO)
c) . Ensure CIP FMT1-4 has been carried out and
d) . Drain inside pipe line DPG
e) . Ensure Exhaust Fan the position OFF

 f) . Finishing / cleaning

2. Dismantling Platform FMT1-4 :

a). Make sure all mechanical part, pipe and electrical instrument demolition has
been carried out.
b). Dismantling Railing, Checker plate / Floor
c). Install the support frame on the tank FMT1-4
d). Dismantling H beam & Legs Platform

3. New install Platform FMT1-4

a) . Marking & Installing Chemical Anchors

b) . Installed Column New Platform
c) . Installed Bracing New Platform
d) . Install the gantry for the tank lifting process
e) . Installed Railing, Floor, Ladder & safety Cage
f) . Installed Protection Area
g) .Modification Ducting FMT 1 ~ 4 2 Units

4. Reinstall Mechanical,pipe, electrical instrument after new platform

installation
FMT 1-2
a) Connect and Termination cable to part and component to tank FMT 1&2
b) Additional Valve and Modification Pipe DPG FMT 1 & 2 and connect to tank
c) Install Pipe Inlet including Connection part to top tank FMT 1 & 2
d) Install Pipe Outlet including Connection part to bottom tank FMT 1 & 2
FMT 3-4
a) Connect and Termination cable to part and component to tank FMT 3 & 4
b) Additional Valve and Modification Pipe DPG FMT 3 & 4 and connect to tank
c) Install Pipe Inlet including Connection part to top tank FMT 3 & 4
d) Install Pipe Outlet including Connection part to bottom tank FMT 3 & 4
FMT 5-6
a) Complete Hot water for Cleaning
b) Complete Platform Grounding and new Grounding Pit
c) Fit & Weld Manual Valve for DPG FMT -6
d) Blind Line Compressed Air with temporary just connect to FMT5 & 6 Line
e) Test AGT, Lamp or all Part And component at Tank FMT 5 & 6
f) Hand Over

Post cleaning:

- Have all objects such as screws, washers, nuts, bolts, tapes, glues, grease,
lubricants and other small items that could potentially contaminate water products
been removed from the work area?

To be completed by the affected department. Yes No N/A

- Did the work lead to changes on the equipment / infrastructure that could impact
food safety/GMP Compliance (e.g. different paint or colour etc.)? If yes please
inform the Food Safety Team/QM Team and/or introduce it to the MOC process.

- Check operation machine. Ensure the machine runs normal.

- Is the equipment, its infrastructures and the environment back in clean status?
