Review the Emergency Use Authorization (EUA) Fact Sheet. This sheet will contain
information on both Pfizer and Moderna vaccinations. Click here to view.

Reminder: In order for immunization to take full effect, you must receive two doses
of the vaccine.

On completion of the 1st dose appointment, use the Scheduling Control button below
to schedule/confirm your 2nd dose appointment. If there is no availability listed
for the 2nd dose, the clinic may not be accepting appointments yet. Please check
