    When a last line of defence is not working severe incidents happen.

    1.Bhopal 3. Ski-lift Italy


    Flare was not working Emergency break was bypassed
    (this was known) (on purpose)

    2. Chernobyl 4. Macondo oil rig


    Emergency stop did not work Blow-out preventor failed
    (design flaws were known)

    ASPECTS
    A solid last line of defence, like SIL rated interlocks, Pressure relieve devices
    and Passive barriers, are essential for scenario’s that involve multiple fatalities.
    Assure high reliability by a good design, preventive maintenance and regular
    testing.
