HEMATOLOGY
BIOCHEMISTRY
The kidneys play several vital roles in maintaining health. One of their most important jobs is to filter waste materials from the blood
and expel them from the body as urine. They also produce and release erythropoietin (EPO), which stimulates the bone marrow to
make red blood cells, renin, which helps control blood pressure, and calcitriol, the active form of vitamin D, which is needed to
maintain calcium for teeth and bones and for normal chemical balance in the body. Among the important substances the kidneys help
to regulate are sodium, potassium, chloride, bicarbonate, calcium, phosphorus, and magnesium. The right balance of these
substances is critical. When the kidneys are not working properly, waste products and fluid can build up to dangerous levels in the
blood, creating a life-threatening situation. Kidney function tests are common lab tests used to evaluate how well the kidneys are
working.
BIOCHEMISTRY
Note
1. In an asymptomatic patient, Non alcoholic fatty liver disease (NAFLD) is the most common cause of increased AST, ALT levels.
NAFLD is considered as hepatic manifestation of metabolic syndrome.
2. In most type of liver disease, ALT activity is higher than that of AST; exception may be seen in Alcoholic Hepatitis, Hepatic Cirrhosis,
and Liver neoplasia. In a patient with Chronic liver disease, AST:ALT ratio>1 is highly suggestive of advanced liver fibrosis.
3. In known cases of Chronic Liver disease due to Viral Hepatitis B & C, Alcoholic liver disease or NAFLD, Enhanced liver fibrosis (ELF)
test may be used to evaluate liver fibrosis.
4. In a patient with Chronic Liver disease, AFP and Des-gamma carboxyprothrombin (DCP)/PIVKA II can be used to assess risk for
development of Hepatocellular Carcinoma.
BIOCHEMISTRY
LIPID PROFILE
1.Triglycerides: When triglycerides are very high greater than 1000 mg/dL, there is a risk of developing pancreatitis in children and
adults. Triglycerides change dramatically in response to meals, increasing as much as 5 to 10 times higher than fasting levels just a
few hours after eating. Even fasting levels vary considerably day to day. Therefore, modest changes in fasting triglycerides measured
on different days are not considered to be abnormal.
2. HDL-Cholesterol: HDL- C is considered to be beneficial, the so-called "od" cholesterol, because it removes excess cholesterol from
tissues and carries it to the liver for disposal. If HDL-C is less than 40 mg/dL for men and less than 50 mg/dL for women, there is an
increased risk of heart disease that is independent of other risk factors, including the LDL-C level. The NCEP guidelines suggest that
an HDL cholesterol value greater than 60 mg/dL is protective and should be treated as a negative risk factor.
3. LDL-Cholesterol: Desired als for LDL-C levels change based on individual risk factors. For young adults, less than 120 mg/dL is
acceptable. Values between 120-159 mg/dL are considered Borderline high. Values greater than 160 mg/dL are considered high. Low
levels of LDL cholesterol may be seen in people with an inherited lipoprotein deficiency and in people with hyperthyroidism, infection,
inflammation, or cirrhosis.

1. TSH levels are subject to circadian variation, reaching peak levels between 2 - 4.a.m. and at a minimum between 6-10 pm . The
variation is of the order of 50% . hence time of the day has influence on the measured serum TSH concentrations.
2. Alteration in concentration of Thyroid hormone binding protein can profoundly affect Total T3 and/or Total T4 levels especially in
pregnancy and in patients on steroid therapy.
3. Unbound fraction ( Free,T4 /Free,T3) of thyroid hormone is biologically active form and correlate more closely with clinical status of
the patient than total T4/T3 concentration
4. Values <0.03 uIU/mL need to be clinically correlated due to presence of a rare TSH variant in some individuals
SPECIAL
The prothrombin time (PT or protime) is the actual time, measured in seconds, for an anticoagulated sample of plasma or whole blood
to clot after it is added to a thromboplastin reagent. Physicians may request a PT to investigate a history of a bleeding tendency prior to
invasive surgical procedures, prior to intravascular cardiac surgery or other procedures that interfere with coagulation, determine the
etiology of abnormal bleeding, assess liver function, and monitor warfarin therapy.

Interpretation
Positive test for SARS-CoV-2 confirms the diagnosis of COVID-19.
Test Indications :
Individuals with suspected COVID-19 infection (As per WHO ‘Case Definition’ –
https://www.who.int/publicationsdetail/global-surveillance-for-humaninfection-with-novel-coronavirus-(2019- nCoV).
Clinical Significance :
COVID 19 could be asymptomatic or present with symptoms of fever, cough, shortness of breath. Severe cases could lead to
pneumonia, severe acute respiratory syndrome and sometimes death. One should seek medical advice if a symptomatic patient has
been in close contact with a person known to have COVID-19 or live in or have recently traveled from an area with ongoing spread of
COVID-19. COVID-19 is declared as a pandemic but need to be controlled by early diagnosis and following standard and droplet
precautions to prevent transmission.
Disclaimer:
1. Incubation period for SARS-CoV-2 infection ranges from 1 to 14 days, and clinical symptoms usually manifest within 5 days.
2. If a negative result is obtained from a patient with a high index of suspicion for COVID-19 virus infection, particularly when only
upper respiratory tract specimens were collected, additional specimens, from the lower respiratory tract if possible, should be
collected and tested.
3. Lab is NABL accredited but COVID-19 is not in scope.
