Unsafe work Procedure-the work sequence was Fractured left 3 shifts
not followed because the lashing crew was up index finger
in the sub level lashing down ore into the box
and at the same time the box tramming was
below emptying the box.
Corrective Action
To be advised after the investigation is
concluded.

 PPE supply was stable during the week except for hard hats and safety belts).
 Cap lamp compliment - 497, Total functional Cap lamps - 555. (Received 50 new cap lamps)
 Total gas testers –16, functional gas testers -13, (3 Faulty).
 Currently mining has a total of 459 safety belts.
 Safety belts in stock – 5.
5.0. Compliance Issues
 One hour reentry assessment is due and the Inspectorate has not indicated a possible date to visit the mine.

6.0. Induction/Training
 7 new employees went through the pre-employment safety induction.
 2 employees from leave were inducted on return to work safety.
 3 contractors went through the pre-employment safety induction.
