AFFIDAVIT OF SUPPORT/GUARANTEE

I, SAMSON C. LINGLINGON, citizen of CANADA of legal age and presently residing in Canada at
Oakville, Ontario after having been duly sworn to in accordance with law, hereby depose and state:

1. That I wish to invite the following person to study in Canada:

2. That I know personally the above-named person who is my friend and distant relatives
respectively.
3. That I hold my self responsible for his accommodation and support during his study in Canada.
4. That I guarantee that he will abide by all laws and regulations of the Canadian Government;
5. That I further guarantee that the above-mentioned will return to the Philippines on or before
the expiration of the period of his study in Canada.
6. That this affidavit is being executed to attest the foregoing facts and not for the purpose of
FRAUD but in connection with the passport and/or visa application of the above-named person.
