7, 1982 at Bacung, Sumisip, Basilan Province, to spouses DUJINAN J.
ANTASAL and ASGAR MUNTASAL;;

2. However, due to inadvertence and/or oversight, his birthdate was erroneously

recorded as OCTOBER 2, 1982 on his Passport issued by the Department of Foreign
Affairs when it should have been OCTOBER 7, 1982 as evidenced by his Certificate
of Live Birth;

3. I hereby certify that, JIMBOY ANTASAL ASGAR with birthdate OCTOBER 2,

1982 and JIMBOY ANTASAL ASGAR with birthdate OCTOBER 7, 1982 refer to
one and the same person;

4. I make these statements under oath and in good faith in order to establish the fact
that his true and correct birthdate is OCTOBER 7, 1982;
