Pls help me to clear my PF amount.

Last week also I was admitted in our hospital for 4 days and discharged on 23.11.23. This PF amount is very useful for my medical expenses.

This has reference to above, I wish to inform you that I have been left and kept under medical management against my illness with our
hospital advice. Because of this I quit my job from our hospital. As I need of funds to face my medical expenses, I have applied for PF amount
through online. My claim was
rejected stating the reason that:-

Form 10c: Claim rejected Date of Joining is 01/2020 WHEREAS CONT RECEIVED FROM 07/2020 CLARIFICATION LETTER FROM ER CALLED.
FORM 19: Claim rejected warning - 520160 MULTIPLE MEMBER ID FOR SAME BANK ACCOUNT, KINDLY VERIFY IN DETAIL BEFORE
PROCESSING.

I have been informed that my PF amount is credited with two member ID account. As my recent service starts from Jan 2020 from with our
hospital, upto Jul 20 employer and employee PF contribution wrongly credited with old member ID (i.e)from Jan 2020 to Jul 2020 contribution
went to previous PF account of mine.

This seven months contribution is wrongly added to PF account with old member ID. Instead of new member ID against fresh appointment of
me with our hospital and created multiple ID with my PF account.
Hence this discrepancy shows in my history slot of PF account, they instructed me to submit clarification letter from employer side to settle PF
amount.

The seven months contribution with old member ID of my account will be eligible to transfer with new member ID of my new account for
which I need your consent and clarification letter from your end.

As I am suffering at chronic illness, I request your esteemed support for release of my amount with PF account, which enable me to face
medical expenses. I will be more grateful to you if you help me at this time of emergency.
