1. What personal action can you commit to avoid an incident like this on your ship?
It is critical to prioritize overall safety of me and the other crews on board, as well as the
well-being of the ship. Implementing strict rules and regulations when performing tasks that
involve enclosed or isolated spaces shall help in preventing unnecessary risks and high chances
of danger or even cause death. By upholding training sessions, seminars, and drills often could
help familiarize the crew to increase their awareness when they are on duty. I will value a strict
risk assessment and wearing proper personal protective equipment before entering an enclosed or
isolated space enhances safety among individuals working. In this way, I can prevent incidents
and accidents that would happen.
2. What are the procedures and controls on your ship that would prevent this from
happening?
Entering an enclosed space is one of the most dangerous work duty that the crew
experiences, due to its high risk of danger brought about different risk and hazards presented in
the workplace. With that being said, procedures should be strict and follows certain rules and
regulation on board. To elaborate, procedure should be documented and follows strict
communication practices. The person above or the person monitoring should always have a close
look out to the person inside. In case of emergency, it is easy to respond and aid the emergency.
When entering enclosed space, crew inside should be aware on the surroundings and should have
a sharp observation on the surroundings. Always communicate and observe properly when
conducting work. In case of emergency, report immediately and discontinue the work and
prioritize safety.
