Subject: 191122-PAR SL03 Engine is damage

1. Facts of the event:

Operator is turning the vehicle but the engine suddenly shuts down. YES staffs tried to move
the Vehicle by other SL to repaired area.

2. Action taken:

We detect that there is water in oil pan. After dismantling the engine, we realize that:
The valve is broken, the cylinder sleeve and cylinder head is damage, piston is crack,
connecting rob is bent

3. Estimated of loss:

- 2 of valve are broken.


- 1 piston is crack.
- 1 cylinder sleeve is crack.
- Cylinder head is damaged.
- Gasket of cylinder head is damage.
- Crank shaft is needed to check for concentricity.

4. Analysis reason of incident:

There is a chain reaction of events. One problem leads to another and


eventually a valve failure. First, the valve is got problem, the pieces for broken valve
fall into the combustion chamber and wreak havoc on the piston and cylinder head.
Then with more force on parts, so piston and cylinder sleeve are cracked, cylinder head
is damaged. Maybe, the Crank shaft is unconcentricity at holder positions.
Valves break in one of two places, where the head is joined to the stem, or
where the keeper groove is machined into the end of the stem. Causes of head
breakage include fatigue failure from valve flexing (from off-square seating that
causes the stem to flex every time it seats), pounding (from excessive valve lash),
stretching (from excessive heat and/or rpm), and thermal shock (sudden temperature
changes as when abruptly shutting off a highly loaded engine).
The groove of valve lock is damage.

Breakage at the stem tip can result from excessive side loading if the installed
stem height is not right and causes rocker arm misalignment. It can also be caused by
excessive valve lash that prevents the valve train from cushioning the valve as it
closes. Another condition that may cause the tip of the stem to snap off is wear or
scratches in the keeper groove(s)
The underlying cause of brake valve is misalignment between the valve guide
and seat. The valve will fail too as flexing causes it to fatigue and break.

3. Lesson and preventive:

Check periodic dimensions, condition of parts for well operation of engine .
