Re: Ms Jane Simms, aged 46

I am writing to discharge Ms Simms, a 46-year-old single lady, living with her sister, who was admitted to
rehabilitation hospital for a Progressive Multiple Sclerosis and she needs your further care.

Ms Simms was diagnosed with Multiple Sclerosis twenty years ago, and she has experienced a
worsening of depression symptoms due to her invalid retirement. She also has suffered pressure sores
on her right buttock and her weight has increased due to her prolonged sitting.

In addition to her usual treatment (ACTH and corticosteroids), Ms Simms was commenced Prothiaden
150mg daily and antidepressant (dosulepin hydrochloride). She is to undergo a low calorie diet, a series
of range-of-motion, stretching and strengthening exercises and, occupational therapy, to ensure a full
recovery.

Ms Simms needs assistance with monitoring her treatment and weight, maintaining skin integrity and
continuing exercise program. We appreciate your support in monitoring her compliance in this regard
and encouraging new activities and interests for her.

Dear Dietician,

Re: Nina Sharman, aged 62

I am writing to refer Ms. Sharman, who had an episode of choking with a piece of food and requires an
urgent assessment of nutritional status and swallowing function due to high risk of aspiration.

Ms Sharman is single and she is under the Australian Guardianship and Administration Council
Protection. She is a new patient of Dementia Specific Unit at Westside aged care facility. Her medical
history includes Ischemic heart disease, osteoarthritis and type 2 diabetes, which has been managed by
diabetic diet for several years. Also, she suffered from stroke in 2011, since then she impaired her gait.

Ms Sharman has no known allergies. She has upper and lower dentures but refuses to wear due to
confusion and disorientation, however, she is able to follow simple instruction. In addition, the patient
appears to have an increase in appetite and progressively gained weight up to 10kg over the last 5
months. Her current weight is 106kg with a body mass index of 30. She complained of chronic
constipation managed with laxatives. Her vital signs and blood glucose levels were within the normal
range.
