Anupul, Bamban, Tarlac do hereby depose and say:

1. That I am the accused in a case entitled PEOPLE OF THE

PHILIPPINES VERSUS docketed as CRIMINAL CASE NO. for
Theft;

2. That I promise and undertake that I will appear and testify in the
court hearing where my appearance is required in connection
with the above-mentioned case;

3. That I am executing this affidavit of undertaking freely and

voluntarily.
