Regarding our research topic of Good Governance and social responsibility, the
researchers would like to conduct interviews with government and business sector professionals
as fourth-year students at Pamantasan ng Lungsod ng Maynila. With respect to the
aforementioned factors, we cordially request your enthusiastic involvement in this survey. Your
forthcoming and candid response will comprise critical information that will significantly
contribute to our research on this subject.

Regarding the acquisition and utilization of the survey's data, the Data Privacy Act of 2012
(DPA) of the National Privacy Commission and any other applicable privacy regulations in the
Philippines shall apply. The responses provided in this survey will be safeguarded with the
utmost confidentiality and exclusively utilized for scholarly and research intentions.

The researcher respectfully requests that you allocate some of your time to finish the
interview. We assure you that without your consent, no third-party user will have access to any
personal information we collect. In addition, as a survey participant, the researchers solemnly
guarantee that it will be contributed to this study without expecting anything in return from the
