Dear Mr. Dindo Estenzo (Project Supervisor)

This letter constitutes official notice of your suspension without pay for One (1)
working days because of your inattentive behavior towards your personnel regarding
safety policies inside the plant, specifically the incident of your personnel going near a
moving forklift inside process line.

It is your job to supervise your personnel to ensure the well being of your
personnel as a secondary safety supervisor with your safety officer.
