1. DEFINITIONS & ABBREVIATION

1.1 Definitions

HAZARD An inherent physical or chemical characteristic that has the potential


for causing harm to people, property, or the environment. In the
word it is a combination of hazardous material, an operating
environment, and certain events that could result in an accident.

HAZARD EVALUATION The analysis of the significance of hazardous situations associated


with a process or activity.

QUALITATIVE RISK ANALYSIS It is a qualitative technique to pinpointweaknesses in the design and


operation of facilities that could lead to accidents.

HAZARD IDENTIFICATION The pinpointing of material system, process and plant


characteristics that can produce undesirable consequence through
the occurrence of an accident.

EVENT An occurrence related to equipment performance or human action,


or an occurrence external to the system upset.

RISK The combination of the expected frequency (event / year) and


consequence (effect / year) of a single accident or a group of
accidents.

RISK ASSESSMENT The Process by which the results of a risk analysis is used to make
decisions, either through relative ranking of risk reduction strategies
or through comparison with the risk targets.

HUMAN ERROR Any human action that exceeds some limits of acceptability (i.e., an
out of tolerance action) where the limits of human performance are
defined by the system. Includes actions by designers, operators, or
managers that may contribute to or result in accidents.
LIKELIHOOD A measure of the expected probability or frequency of an event’s
occurrence.

2. FACILITY DESCRIPTION
The main purpose of the rental heater packages facility is to partially treat the crude oil received
from the production wells, increase the crude temp. up to set point to separate water and gas
from the crude oil (three phase heater) then send it to wash tank and then to oil receiving tank
which send it to NORPETCO storage tank via pumps.
The produced gas is sent to the cold vent and the produced water is sent to skimmer tank then to
lined drain pit.
Rental Heating Package area are:

3. STUDY SCOPE AND OBJECTIVE


NORPETCO assigned MIS to carry out a Quantitative risk assessment study (QRA) for ABRAR
STATION located in western desert which includes Rental Heating Package, storage tanks, gas
boots and production office.

3.1 OBJECTIVES
The objective of the QRA is to calculate the overall risk to demonstrate that the risk to employees
and contractors (Individual Risk) and members of the public (Societal Risk) is acceptable and
reduced to a level which is As Low As Reasonably Practicable (ALARP) by recommending risk
reduction measures to reduce risk, where appropriate, and to allow a comparison of the risk with
the Risk Criteria.
The Quantitative Risk Analysis QRA process involves generating numerical estimates of the
consequences associated with the potential hazards and the likelihood of occurrence or frequency
of each major accident hazard. The potential consequences are combined with the frequency to
yield the numerical levels of risk. Since the process considers a large number of combinations, it
is usual for the majority of the calculations to be carried out using proprietary software and spread
sheets.
3.2 SCOPE OF WORK
The scope of Quantitative Risk assessment covers all the process / utility units within ABRAR
station as follows:

 Rental Heating Package H-100

 Storage tanks TK-01 till TK-10

 Gas boots 1,2 and 3

 Chemical skids

 Production office

 Caravan

 Generator

 Fire Room

A Quantitative Risk Assessment is a study mainly aimed at evaluating the risk to which plant
personnel and offsite population are exposed, because of the hazards present in an industrial
installation.
Also risks to the environment and plant asset integrity, with the relevant economic losses, may be
included in a QRA, or specifically addressed in separate documents.
The QRA performed for this Project is fully probabilistic, i.e. both the frequency and the
consequence of the identified accidental events are quantitatively evaluated. The originating loss
of containment events are identified as “random ruptures”, occurring for several causes,
independently of possible deviations of process parameters.
The results of the analysis are presented in the form of iso-risk contours, reported on the map of
the plant and surrounding area. In particular, the parameter reported on the map is the LSIR, i.e.
the maximum theoretical risk to which can be exposed an individual at a given location.
In addition to the LSIR, also the individual risk to specific worker groups is evaluated, often
referred to as IRPA (Individual Risk Per Annum). The IRPA takes into account realistic exposure
patterns of an individual to the hazards, e.g. the presence time duration at several locations, the
permanence outdoor or indoor, the availability of personal protective equipment, etc.
Furthermore, the total IRPA should include not only the risk contribution deriving from loss-of-
containment events, but also the contribution related to other occupational hazards.
The QRA is carried out according to the methodology and assumptions illustrated in the attached
Assumption register (APPENDIX A).
The risk model has been implemented in a PHAST RISK file version 8.71

4. METHODOLOGY
The QRA methodology indicates a stepped approach to evaluate the Quantitative risk assessment
for the plant. The study approach has followed internationally accepted risk analysis practice
covering the following steps shown in figure 3:

The methodology for the QRA could be summarized as follows:


1- Identification of potentially LOC, which includes:

- Identification of scenarios
- Data acquisition
- Determination of isolatable sections with the relevant equipment and inventory which are reported
in Appendix D with the relevant released substance, pressure, and temperature.

- Hazard identification and Determination of Failure Events

2- Incident Scenarios consequences assessment, which includes the following:

- Modelling of Gas leaks and dispersion clouds


- Modelling Jet fires, pool fires, Flash fires
- Modelling Explosion overpressure
- Estimate the Impact and impairment criteria.
- Evaluation of the consequence

3- Assessment of the frequencies related to the different LOCs, which includes the following:

- Determination of release frequencies: Calculation of the Leak Frequency for each release point, for
leak with nominal hole size of 5 mm, 25 mm, 100 mm, and 150 mm.
- A Parts Count of the items in each isolatable section is performed, considering the equipment types
defined in the reference database used for obtaining the base leak frequency data. The total leak
frequency calculated for an isolatable section is then allocated to the release points of the section.
The Parts Count with the Leak Frequency summary are reported in Appendix E.

4- Calculation Assessment of escalation potential on Personnel and Personnel risk.

- The output of Phast Risk relevant to Outdoor/Indoor Vulnerability is reported in Section 8.6
- Calculations of the Exceedances Maps for selected radiation and overpressure thresholds. The
results of the Personnel risk assessment are presented as frequency contours of exceeding a given
value of radiation or overpressure, laid over the plant plot plan. The exceedance maps allow
identifying the areas where the likelihood of impairing plant Personnel exceeds the limits defined in
the risk acceptance criteria. The maps are reported and discussed in Section 8.6 of the report.

6- Evaluation of the recommendations of the QRA through a cost benefit analysis by comparing the
Cost of implementing the recommendation benefits of applying the recommendations versus the

5. IDENTIFICATION OF POTENTIALLY LOC


The Scenarios identification was carried out, both historical analyses and structured review
techniques, to identify the worst credible accidents likely to occur in the facility during its life-time.
The historical analysis of the incidents has allowed identifying causes, consequences and damages
to asset related to the incident under investigation. The scenarios of interest are the maximum
credible and representative incidents with the potential to cause important damage to the
installation.
The structured review has identified the equipment and lines containing potentially hazardous
materials and to define representative hazardous scenarios. Thus, isolatable sections have been
identified on all process units, according to the Process Flow Diagrams (PFD’s) and Process and
Instrumentation Diagrams (P&IDs).
Some assumptions have been made for this study. They are summarized in APPENDIX A of this
document

5.1 HAZARD SCENARIOS INVESTIGATED

The release of flammable gas or liquid can lead to different types of fire scenarios. These are
dependent on the material released, the mechanism of release, and the operating conditions of the
material, ambient conditions and the point of ignition. Types of scenarios generated due to any
release are:
- Un-ignited Vapor Release / Flash fire
- Jet fire
- Pool fire Vapor cloud explosion
- Boiling liquid expansion vapor explosion (BLEVE) or fire ball

5.1.1 UNIGNITED VAPOR RELEASE - FLASH FIRE


Flash fire is a non-explosive combustion of a vapor cloud resulting from a release of flammable
material into the open air. Released hydrocarbon-air mixture is likely to lead to flash fire on ignition
within the LFL plume. The damage associated to flash fires is mainly related to personnel while
structure and equipment are not significantly damaged by flash fire due to the very low duration of
the phenomenon.

5.1.2 IGNITED RELEASE-JET FIRE


Fire scenarios are likely to be dominated by momentum driven gas jet fires from Pumps. Liquid jet
fires are also likely to occur from separators, accumulators, tanks …etc. These fires were highly
directional in nature depending on leak hole geometry and may extend several tens of meters in
length depending on hole size and release pressure. The flame shape was dependent upon release
characteristic (hole size, shape, release rate and pressure, etc.) and interaction with obstacles. Jet
fires impose a very serious conditions on impacted structures and equipment due to the high heat
fluxes produced in the intense flame envelop and the erosive nature of a high momentum stream.
5.1.3 pool fire
Pool fires were largely limited to the heavy condensate process equipment, storage tanks and diesel
tanks containing substantial amount of liquid hydrocarbon inventory capable of producing sustained
pool fire.
5.1.4 VAPOR CLOUD EXPLOSION (VCE)
Explosion scenarios were not systematically considered for each release scenario but were selected
and associated with the presence of potential explosion sites (PES). PES are plant areas where a
significant level of congestion and/or confinement may be experienced; if a PES is reached by a
flammable cloud, a vapor cloud explosion can occur in case of ignition. In the case of project design,
PES was identified on the basis of detailed equipment layout, piping arrangement or comparisons
with similar assemblies. Typical examples of
PES may be congested process modules, compressors shelters, stacked piping and vessels, piled
structures etc.
5.1.5 BOILING LIQUID EXPANDING VAPOR EXPLOSION (BLEVE)
A BLEVE occurs when a vessel containing liquefied flammable gas under pressure is subjected to
direct flame impingement or flame engulfment which could be applicable on hydrocarbon vessels
equipped with no blow-down system or full flow PSVs.

5.2 IDENTIFICATION OF ISOLATABLE INVENTORIES

Generally, in case of gas release or fire detection, the activation of both shutdown and blow down
systems can successfully control the fire and the gas leak, considering that the process system was
depressurized only in case of fire by isolating the equipment using shut down valves and the
opening of blow down valves to discharge the trapped gas to the flare.

The QRA study considered the isolatable section only to identify the hazardous inventory for each
process section, without any consideration for the activation of both emergency shutdown / blow-
down system which could minimize the consequences of the hazardous scenario if activated early.
The PFDs and P&IDs for each unit were reviewed to identify the hazardous inventories and
appropriate start and end points. The isolatable inventories were identified using the following
ruleset: each inventory ended at:

- An isolation valve / ESD valve (if applicable)

- A relief / blow down valve.

- A normally locked / closed valve or spectacle blind.

The trapped inventory is calculated based on the shutdown philosophy (i.e., trapped volume + peak
flow rate during 2 minutes of shut down) of the plant .

6. INCIDENT SCENARIOS CONSEQUENCES ASSESSMENT

6.1 LEAK SCENARIOS

The consequence analysis is based on a set of representative leak scenarios according to the process
area data and conditions.
For the purpose of the consequence assessment, the higher value of the operating pressure range is
used in the calculations, as this will give a higher release rate and hence make the analysis
conservative.

6.2 STREAM DATA

The first stage in identifying the actual leak flow rates was to define the representative stream data.
The representative stream data were then combined with the hole sizes discussed below to
calculate outflow rates.
For scenarios where both gas streams / condensate stream is represented a two-leak scenario will
be developed to assess the consequence from both streams and then the worst-case stream will be
considered in the consequence assessment.

6.3 HOLE SIZES

A set of representative release hole sizes was assumed for each scenario, as summarized in
APPENDIX A of this document.

6.4 CONSEQUENCE ASSESSMENT

Physical effects from Incident Scenarios, such as heat radiation and explosion over-pressures have
to be calculated to assess damage to Personnel. The aim is to identify which parts of the facility may
be exposed for each potential Incident Scenario and the extent of that exposure.
This exposure will then be used to estimate the potential for escalation, calculate the risk and
contribute to decisions on the need to reduce such risks.
Consequence assessment of the incident scenarios will be carried out using internationally accepted
methodology and software. The software that will be used for the assessment is DNV PHAST Risk
8.71
The consequence analysis results will be expressed in terms of:

 Distance to lower flammable limit (LFL) (gas cloud)


 Distance to Upper flammable limit (UFL) (gas cloud)
 Distance to 50% lower flammable limit (LFL) (gas cloud)
 Thermal radiation distances to specific heat flux levels Kw/m2 (4, 12.5, 37.5 kw/m2)
 Explosion exceedance contours

The first step is to define the potential event sequences and potential Hazard Incident. Accidents
begin with a Hazard Incident, which usually results in the loss of containment of material from the
process.
Typical Hazard Incidents might include a hole in a tank or pipe, runaway reaction, fire external to a
vessel, etc.
Once the Hazard Incident is known, source models are selected to describe how materials are
discharged from the process. The source model provides a description of the rate of discharge –
liquid, vapor, or a combination.
Dispersion models convert the source term outputs to concentration fields downwind from the
source. A dispersion model is subsequently used to describe how the material is transported
downwind and dispersed to some concentration levels.
For flammable releases, fire and explosion models convert the source model information on the
release into energy hazard potentials such as thermal radiation and explosion overpressures.
Both gas and liquid releases are considered in this study and the released materials can pose a
flammable hazard. Any initial release may form a liquid pool or a liquid or gas jet, depending on the
release pressure and material phase.
For a liquid pool, some vapors may flash on release or may be generated by evaporation from the
subsequent liquid pool formed. The hazard events identified are modelled as part of consequence
analysis to determine the severity of the accident in terms of causing human fatality.

7. IDENTIFICATION OF IMPACT / IMPAIRMENT CRITERIA


Fire heat radiation, flammable mass and explosion over pressure were evaluated referring To
recognized damage threshold values here below for specific accidental scenarios (e.g. jet fire, flash
fire, etc.), these limits are listed and used in the study.

7.1 FIRE HEAT RADIATION

For the assessment of fire heat radiation within the QRA The Probit method was used.
For The Probit Method, The Ellipses Cover A Range Of Lethality Levels, Calculated From The
Radiation Intensity And Exposure Time As Specified In The Flammable Parameters.

7.2 FLASH FIRE

For the assessment of flash fires within the QRA it has been assumed that personnel within the
flammable cloud (100% LFL) at the time of ignition will result in 100% fatalities.

7.3 EXPLOSION PEAK OVERPRESSURE

Explosions are due to the accumulation of flammable gases/vapors in a confined/congested volume


then ignited. It is assumed that the explosion volume is filled with worst stoichiometric
concentration.
In order to assess the damage derived by an explosion a detailed evaluation of the damage effects
of explosions over Personnel indoor and outdoor should take into account, besides the overpressure
intensity, also the following factors:
 Drag force (especially on small objects like pipes)
 Impulse
 Type of target exposed (pressure/atmospheric vessels, piping, connections, structural elements,
buildings) and relevant dynamic response to the load.

In the QRA, the following impairment criteria of the assets subject to blast overpressure
(Vapor Cloud Explosion and BLEVE) was considered which is derived from TNO purple book
“Guidelines for quantitative risk assessment “

8. PERSONNEL DISTRIBUTION
In order to calculate the Individual Risk to personnel, population data is required which identifies
the distribution of personnel on the site. The manning data for the plant was assumed as there is no
confirmed data, There are 3 non-equal shifts per day on the process plant.
The manning distribution data for the project is attached in Appendix A QRA assumption register.

9. FREQUENCIES ASSESSMENT
The starting point for the risk model is a list of major accident events along with their associated
frequency of occurrence (the release frequency). This section outlines the frequency estimation for
the risk assessment.
The frequency of the hazardous incidents is estimated using the OGP database. This is used for
vessels, piping, valves, and pumps, etc.
A base data set for each of the identified major accident event is developed. Using this base data set
developed, the frequency of an initiating event calculated by a combining the generic data set and
the equipment parts count. The parts count is undertaken from the P&IDs, to estimate the number
of potential leak sources.

9.1 PROCESS RELEASE FREQUENCIES

Process release events involve the loss of containment of process material i.e. gas or liquid. Release
frequencies were estimated by the following generalized relationship:

 Release frequency = Ʃ (Leak frequency per component) x (Parts count for component type)

Where: Ʃ = the sum for all equipment associated with a Major Accident Event
Component failure frequencies are listed in OGP databases.

9.2 PART COUNTS

A part count was conducted for those plant areas, which are related to a potential release event due
to equipment leak. The count was broken down by the inventory involved and by section of the
process in order to match each particular major accident event.
Part counts were done from the P&IDs and plot plan. The part count involves the followings:
 Vessels;
 Valves;
 Pumps;
 Small bore connections;

Typically release sources were associated with the main equipment/item present within the
isolatable inventory. For dislocated inventories located in different areas, multiple release sources
were defined, and each one will be placed along the displacement of the inventory at a predefined
distance.
An estimate will be done for pipe lengths based on plot plans for each pipe diameter in the major
accident events. The measured pipe lengths on the plot plan were multiplied by a factor of 1.25
(increase of 25%) to allow for pipe runs in the plant.
The selection of the leak frequencies database is a key aspect as it relevantly affects the overall risk
results. The parts counts will be combined with the release frequencies from the OGP database to
produce a frequency of release by hole size for each process stream.
The following table-5 summarizes the leak frequency for each LOC scenario from each isolation
section:

9.3 INCIDENT SCENARIOS FREQUENCIES ASSESSMENT

LOCs do not necessarily cause damage to asset. The development of the LOC into a serious incident
(Incident Scenarios) depends on the effect of mitigating factors, e.g. if immediate ignition of a gas
release occurs, fire detectors can activate shutdown and deluge systems prior to further escalation.
The formal diagrammatic and systematic presentation techniques for the development of events
into incidents are Event Trees. An Event Tree will be constructed by laying out each identified
contributing factor in chronological order and assessing all possible combinations of factors to
provide a number of possible hazardous event outcomes.
The lines connecting the contributing factors correspond to all the possible paths that can occur
during the development of the physical phenomenon. Each branching along the path shows either
the possibility of two different “spontaneous” outcomes or the occurrence of success or failure in
mitigation of the incident.
The Top Event on the left side of the event tree is the release of flammable hydrocarbon from a
process unit. For each top event, the base expected frequency of occurrence will be calculated
through the part count technique. On the right side of the tree, the top event develops, through the
subsequent branches, a number of outcomes. The frequency of each single outcome is equal to the

outcome likelihood is again expressed as a frequency (i.e. occurrences per year).
During modeling the study in DNV PHAST, the event tree will be developed embedded in the
software.

9.4 IGNITION PROBABILITIES

The total probability of ignition is established based on OGP Risk Assessment Data Directory, Report
No. 434-6.1, “Ignition Probabilities” which summarizes the data in TNO Purple Book, Guidelines for
Quantitative Risk Assessment, December 2005.
Table-6 shall summarize the probability of ignition for all main ignition points foreseen in the
complex, respectively.

10. RISK ANALYSIS


Performing a QRA is a very calculation intensive process hence it is usual for a number of software
and spreadsheet-based tools to be used for various stages of the overall QRA.
It is usual within a QRA to generate a range of risk measures. The following summarizes the
measures of risk generated for this assessment:
 Location Specific Individual Risk (LSIR) : The risk at a particular location for a hypothetical individual
who is positioned there for 24 hours per day, 365 days per year.

 Individual Risk Per Annum (IRPA) : The level of risk (of death) experienced by an individual person.
This measure of risk takes into account the amount of time a person is exposed to potential major
accident hazards. The individual risk therefore includes both the proportion of time onsite and also
the proportion of time in specific locations on the facility where they may be exposed to the effects
of potential hazards. IRPA is independent of the number of people exposed.

 Potential Loss of Life (PLL) : The level of risk (of death) experienced by the whole group of people
exposed to the potential major accident hazards.
For workers, the Risk Tolerability Criteria to be used for Individual risks are extracted from the Risk
to People (Operations) Assessment Matrix (HSE Risk Management and Reporting).
For IRPA index, the tolerability criteria is compared with risk figures shown in the “row” 4 (single
fatality) of the Risk to People (Operations) Assessment Matrix.

11. RISK RESULTS ANALYSIS


11.1 LSIR (LOCAL SPECIFIC INDIVIDUAL RISK) MAPS
This parameter represents, for each location on-site and off-site, the aggregated frequency of
occurrence of conditions that can cause fatal effects on an exposed individual.
The LSIR is represented by iso-risk contour plots displayed on plant layout; LSIR contours refer to
outdoors conditions.
LSIR takes into account:
 Frequency of the release events;
 Likelihood of specific conditions determining the outcomes (including e.g. ignition probability,
weather conditions, wind direction probabilities, etc.);
 Outdoor Vulnerability (i.e. the fatality probability related to physical effects such as heat
radiation, overpressure or toxic concentration).
The LSIR is the risk at a particular location for a hypothetical individual who is positioned there for
24 hours per day, 365 days per year.
The LSIR contour map is reported in the following Figures, for all scenarios evaluated in the QRA.

11.2 INDIVIDUAL RISK PER ANNUM (IRPA)

Individual Risk per Annum (IRPA) is generally defined as the risk of fatality to a named individual,
belonging to a working category present in the installation, or a generic member of the public off-
site.
IRPA values have been calculated for several workers groups, as per data provided by the Company
and reported in the Assumption Register.
The data used and the calculated IRPA are summarized in Table 9, respectively for the Building Area,
and the Process/Utilities areas.
The tables report:
 The worker group;
 The plant locations (RRPs, risk ranking points) where the member representative of the worker
group is assumed to be prevalently located;
 The LSIR calculated, for outdoor and indoor exposure, at the RRPs
 For the two considered shifts (day/night): the duration (hours) of the shift, the number of
individuals belonging to the worker group, the percentage of presence outdoor and indoor.

12. RECOMMENDATIONS
The main purpose of the QRA was to verify if ABRAR STASTION is suitable to ensure that, in case of
an accidental event, no significant escalation is foreseen between different Process areas or on
external facilities leading to fatalities.
The Potential loss of life at ABRAR STATION has been checked as well.
The results of the analysis are presented in a format suitable for providing evidence of compliance
with risk acceptance criteria and supporting decisions for a cost-effective implementation of risk
reduction measures.
As long as the risk to operators’ groups is within ALARP, the preliminary conclusions arising from the
QRA results are summarized in the following:
1- Ensure there will be Lightening protection.
2- Ensure that instrument critical loops are SIL classified.
3- Ensure that safety distance had been considered as inter spacing and external spacing (with
respect to the prevailing wind).
4- Consider adjacent area for future expansion (if any).
5- Safety and emergency procedure related to activities of projects construction and field
operators in the plant shall be considered also the contractor management and emergency
procedure shall be evaluated as to control the risk during emergency cases.
6- Training of workers for the emergency response plan in case of fire or gas leak specially in
process area.
7- Strict application of the effective permit to work system.

8- The Group with highest value of IRPA and PLL is the HSE team and control room team with
5.08E-06 & 4.11E-06 IRPA respectively which is considered in the lower part of ALARP .

Subject Identification of potential LOCs

“Random rupture” analysis will assume that loss of containment events (LOCs) can
affect all the process items of the installation containing flammable and/or toxic
substances, due to any kind of cause, and estimate the relevant expected frequency
from leak databases that collect and elaborate the historical experience on past
accidents, as detailed in the following sections.

All the releases potentially leading to an acute damage to an exposed individual shall
be evaluated in the QRA. In particular, the following scenarios shall be assessed:
Hydrogen sulfide dispersion, flammable dispersion, jet-fire, pool-fire, flash-fire, vapor
cloud explosion, vessel burst (as a result of BLEVE).

Subject Release Cases

The following release cases, defined in terms of equivalent hole size diameter, will be
analyzed:

Onshore facilities leak frequencies assessment


• Small: 5 mm;
• Medium: 25 mm;
• Large: 100 mm;
• Full bore: 150 mm.

The largest rupture class has been limited to a hole of 150 mm. In fact, since the
HCRD/OGP leak frequency database does not allow a differentiation within the class
“from 100/150 mm to Full bore”, the choice of the upper bound (Full bore diameter) as
representative of very large ruptures seems exceedingly conservative, unless a specific
evaluation and justification is carried out.

Subject Parts Count and Isolatable Sections


A parts Count shall be performed to calculate the leak frequency of the Isolatable
Sections.

The following main assumptions shall apply in the Parts Count methodology:

 Areas of the plant are broken down into Isolatable Sections, in order to manage the
emergency depressurization of the plant and to limit the inventory that may feed a
release. Section isolation boundaries in parts count shall be defined by:

- Normal closed / open valve and check valves


- Rotating equipment (compressors / pumps)

A parts count is associated to each Isolatable Inventory, but the analysis is further divided
to consider all the potential release scenarios within each inventory (e.g. in a typical two-
phase separator specific parts counts shall be performed for both gas and liquid phase
separately).

Typically release sources shall be associated with the main equipment/item present
within the isolatable inventory. For dislocated inventories located in different areas (e.g.
pipes within piperack), multiple release sources shall be defined, and each one shall be
placed along the displacement of the inventory (i.e. length of the piperack) at a
predefined distance (i.e. tentatively one release source every 10 m).

The Parts Count will make reference to the type of equipment defined in OGP datasheet,
and consider all the items shown on the P&IDs. The length of the piping shall be
determined based on the isometric drawings (when available), or estimated from layout
information.

Leak frequencies for “random rupture” analysis shall be obtained from the following
references:

 OGP Report n.434, in particular: Report n.434-1 (Process release frequencies) and
Report n.434-3 (Storage incident frequencies), covering most of the equipment items;

Subject Release Location and Orientation


The level of detail of the risk distribution depends on the number and location of the
release points selected to represent the risk sources identified in the installation.
In general, an appropriate compromise between oversimplification (too few representative
points) and extreme detail should be sought.
In the QRA, a few representative release points will be selected for each isolatable
section, trying to preserve the main differences in the process conditions (pressure,
temperature, composition, and phase) and the location of the items belonging to the
section.
The release points will be generally located at 2 m height (in the onshore plant), unless
this assumption is in open contradiction with the geometry and spatial arrangement of the
isolatable section.
For onshore pipelines the release elevation shall be set at ground level.
Release direction depends on the geometry of the system. The following table is
applicable to all hole sizes.

Horizontal gas releases shall be assumed free or impinged, depending on the location of
the release source.
It is observed that an impinged jet will generate a shorter and wider jet flame, and a larger
flammable mass.

The main nodes of the Event Tree are relevant to the probability of ignition
(immediate/delayed).

The following criteria shall be assumed for ignition probabilities assessment:

 Total ignition probability shall be split 50:50 between early and delayed ignition;
 VCE is considered as a credible scenario if ignition occurs in a congested and/or
confined area, otherwise only flash fire is considered for delayed ignition;
 For credible VCE, in case of delayed ignition a 60:40 distribution between flash fire
and explosion is assumed.

Subject Consequence Assessment - Discharge

The discharge from ruptured equipment is a transient phenomenon, where the release
rate is decreasing with time, as a function of the initial conditions, inventory and hole size.
The transient feature of the release is more evident in gas systems than in vapor/liquid
two-phase systems.

The need of including a complete transient modeling of the discharge in the consequence
assessment, or rather selecting some simplifying assumption, depends on the aim of the
analysis and the degree of detail requested.

In the QRA, the discharge modeling shall be carried out according to the following rules:

• The peak flow rate, and the subsequent dispersion, is used to estimate the flammable
mass that may participate to an explosion; the peak flow rate is also used to estimate the
pool-fires (equilibrium model, i.e. early pool-fire in PHAST)

Subject Explosion model

All potential VCEs shall be modelled with the Baker-Strehlow model.

In particular, the following assumptions are applied in the implementation of the method in
the software Phast-Risk.

1. The explosions are centered in the obstructed areas identified in the plant
2. The method implemented in PHAST RISK 8.61 uses the latest Baker-Strehlow-
Tang curves published in literature
3. The obstructed areas are characterized with the following parameters:
A. Degree of confinement: 2.5D (Typical for process modules)
B. Degree of congestion: MEDIUM (typical volume blockage ratio 5%)
C. Reactivity of the substances: CALCULATED by the program based on the
laminar burning velocities of the mixtures (Note: the program calculates a
MEDIUM reactivity for most of the mixtures. This is conservative for streams
made up essentially of methane, which alone would be classified as low
reactivity, but in mixtures slightly exceeds 0.45 m/s, that is the flame speed
value used in BST to classify low reactivity)
D. Any two obstructed volumes separated by less than 20 m are combined in one
explosion site.
A. The flammable mass deriving from the dispersion is calculated considering
Horizontal impinged jet release in process areas: this maximizes the flammable
mass calculated
4. The flammable mass participating to the explosions is located and calculated:
A. Performing the geometric intersection between flammable clouds and explosion
sites
B. Forcing the dispersion flammable mass to fill the relevant explosion site as far
as possible
(i.e. up to a maximum represented by the product of geometric volume of the
explosion site and the stoichiometric concentration of fuel), when the release is
inside a PES
5. A correction is introduced to account for the ground reflection of the blast wave (the
original BS method was developed for aerial explosion). The correction includes a
factor 2 applied to the explosion energy, and a variable factor calculated as a
function of the elevation of the explosion applied to the peak overpressure.

It is remarked that the BS method does not calculate the blast loads deriving from
unobstructed portions of the flammable cloud. This could lead to an underestimation of the
positive phase duration of the blast wave, which is longer for small blast strength (typical
values are 30-80 ms for strong explosion, and 100- 150 ms for weak explosion).
