PETITION FOR ADOPTION

That we, Spouses LUIS AND ESPERANZA JORNACION,


Filipinos, of legal age, married on 23 December 1998 and residents
of Cottage 1, Hemlock St. Sun Valley Estates, Barangay San Juan,
Antipolo City, after having been duly sworn in accordance with the
law, hereby depose and say:

1. That I, Luis Jornacion, was born on __________________ at


________________________. My mother is _________________, a
Filipino citizen at the time she gave birth to me, and my father
is ____________________, a Filipino citizen at the time of my
birth. Copy of my Certificate of Live Birth issued by
_________________ (name of issuing office) is hereto attached as
Annex "A".

2. That I, Esperanza Jornacion, was born on ________________ at


______________________. My mother is ______________________, a
Filipino citizen at the time she gave birth to me, and my father
is _____________________, a Filipino citizen at the time of my
birth. Copy of my Certificate of Live Birth issued by the
_______________________(name of issuing office) is hereto
attached as Annex "B".

3. That the child we sought to adopt is Ayezalyn Remeticado


Pramoso born on July 3, 2008 at _________________. The birth

parents of the child are Antonio Pramoso and Evelyn
Remeticado Pramoso. Copy of child’s Certificate of Live Birth
issued by the Philippine Statistics Office is hereto attached as
Annex "C".

4. Petitioners Esperanza and Luis have been married since 1998


but they remain childless. The spouses have been in custory of
Ayezalyn when she was a one year old, to whom they have
given their love, care and affection.

5. That because Ayezalyn have been showered with love and care
of the Petitioners Esperanza and Luis since she was one year
old, she is very much willing to be the legal daughter of herein
Petitioners Esperanza and Luis.

6. Ayezalyn’s biological parents, Antonio Pramoso and Evelyn


Remeticado Pramoso decided to have Ayezalyn be adopted by
herein Petitioners because of financial difficulties;

7. That for the past fourteen (14) years, Antonio and Evelyn have
witnessed that the Petitioners have truly and sincerely loved
and cared for Ayezalyn like their own child, they have decided
to have her legally adopted by the Petitioners.

8. That adoption social worker, (name of social worker), prepared


the Social Case Study Report (SCSR) dated (date of SCSR) and
approved by (name and position of signatory, if applicable) of
(name of agency). The SCSR is hereto attached as Annex “D”.

9. That the following persons have given the consent to the


adoption:

a. Ayezalyn Remeticado Pramoso, the child to be adopted.


b. Antonio Pramoso and Evelyn Remeticado Pramoso, the
biological parents of the child.

10. That we are financially, physically, and morally capable,


hence in the best position to rear and provide for the needs of
the child as evidenced by the attached documents.

11. That we possess all the personal qualifications required


under the Republic Act No. 11642 and we are qualified to
adopt.

12. That the following documentary requirements form part


of this Petition, hereto attached as;

incapacity of the Spouses Jornacion;
Annex F-12 Letters attesting to the character and general
reputation of Spouses Jornacion from at least
three (3) non-related character references;
Annex F-13 Recent 5R (127 x 178 millimeter) close-up and
whole-body pictures of Ayezalyn;
Annex F-14 Recent 5R (127 x 178 millimeter) close-up and
whole-body pictures of the Spouses Jornacion;

13. That we shall appear before the (RCPC/NACC) to prove
and strengthen our fitness and qualification to adopt on the
date to be determined by the latter.
14. That it is for the best interest of said minor if she would
be considered as the child of herein petitioners because her
financial, spiritual, and emotional needs would be taken care
of and her future would be safeguarded.

15. That, at present, the subject minor is now fourteen (14)


years old and has been with the Petitioners for thirteen (13)
years now. She has adjusted well in the atmosphere and
environment of her foster family. She is observed to be an
active, healthy, sociable and affectionate child. Generally
speaking, her total growth and development is within the
normal level which could be attributed to the positive,
alternative, and stimulating activities given by the foster
family;

16. That the minor sought to be adopted does not own any
real, personal, tangible and intangible property; and

17. That this adoption is for the best interest and security of
the said minor if she would be considered as true and legal
child of herein petitioners and it will make her life normal,
productive, and good persons in the community;

18. That the grant of this Petition shall create a legal status
to the already existing parent-child relationship and shall
redound to the best interest of the child, as she may then
enjoy the benefits of a legally adopted child as sanctioned by
the laws.

PRAYER

WHEREFORE, premises considered, it is respectfully prayed


for the Executive Director of the National Authority for Child Care,
upon notice and publication, that an Order of Adoption be issued
declaring Ayezalyn Remeticado Pramoso for all legal intents and
purposes, our legitimate child, and hereby directing that Ayezalyn
Remeticado Pramoso shall be known as Ayezalyn Pramoso
Jornacion.

Other reliefs just and equitable under the circumstances are also
prayed for.
